INTEGRATION PRINCIPLES. 7.1 In consideration of the mutual benefits and obligations under this Agreement, We will work together to perform the obligations set out in this Agreement and, in particular, achieve the Integration Objectives and, subject to and in accordance with the provisions of this Agreement, We will [local circumstances may lead to one or more of the points below being more appropriately expressed as Integration Objectives and included at clause 6.1 above]: (a) in relation to shared vision and delivery of system outcomes: (i) commit to delivery of system outcomes in terms of clinical matters, Service User experience and resource allocation; (ii) develop and participate in the risk reward scheme where all share in savings generated by reduction in acute activity; (iii) commit to delivering the best possible care for the whole population; (iv) adopt an uncompromising commitment to trust, honesty, collaboration, innovation and mutual support; (b) in relation to working together: (i) commit to work together and to make system decisions on a Best for Service basis; (ii) establish an integrated collaborative team environment to encourage open, honest and efficient sharing of information, subject to competition law compliance; (iii) adopt collective ownership of risk and reward, including identifying, managing and mitigating all risks in performing our respective obligations in this Agreement; and (iv) co-produce with others, especially service users, families and carers, in designing and delivering the Service, (c) in relation to decision making: (i) take responsibility to make unanimous decisions on a Best for Service basis; (together the “Integration Principles”). 7.2 Over the Term of the Agreement, the actual provision of Services will alter on the basis of the most effective utilisation of [staff, premises and other resources (in terms of cost and quality)] and whilst there will be co-operation as to the service design this will not preclude competition between Us in respect of service provision as is needed to achieve the Integration Objectives and which will be reflected in the Services Contracts and changes to those Services Contracts. 7.3 The Primary Medical Providers must not do anything or refrain or delay from doing anything that would put the ACO in breach of the ACO Contract. The Primary Medical Providers must co-operate with the ACO to facilitate the delivery of the ACO Contract.
INTEGRATION PRINCIPLES. 4.1 The Integration Principles agreed by Us are to deliver sustainable, effective and efficient health and social care services with significant improvements over the term of this Agreement. In particular We have agreed the following Integration Principles for the Services: a. High quality care and support is delivered that is personalised, joined up and coordinated around the person b. People will be more in control of their own health and wellbeing c. Safer and stronger communities are built which are more able to meet their own needs d. Primary care is sustainable and is the fundamental building block upon which integrated health and social care is delivered e. Progressive and impactful integration overcomes fragmentation, and resources are deployed to where they are most needed f. The focus of service delivery changes from the current emphasis on the management of illness to an approach based on early intervention, prevention, self-management and choice g. Care is delivered in the right place at the right time by the right person, every day of the week, enabling care and support to be delivered wherever possible close to people’s homes rather than in hospital h. Staff will be given the autonomy and time to care in a system which places a greater emphasis on helping people devise solutions that fit their needs rather than the needs of organisations.

INTEGRATION PRINCIPLES. 7.1 We will work together to achieve the Integration Objectives acting in accordance with the Integration Principles. 7.2 The Integration Principles are: (a) to act with and commit to trust, honesty, collaboration, innovation and mutual support; (b) to make system decisions on a Best for Service basis; (c) to establish an integrated collaborative team environment to encourage open, honest and efficient sharing of information, subject to competition law compliance; (d) to take collective ownership of risk, including identifying, managing and mitigating all risks in performing our respective obligations in this Agreement; and (e) to ensure that Service Users, and other relevant persons are involved to the extent required by Our contractual obligations in Our consideration and implementation of changes to the way Services are delivered. 7.3 Over the Term We acknowledge that We may agree that the actual provision of Services may alter on the basis of the most effective utilisation of [staff, premises and other resources (in terms of cost and quality)] and whilst there will be co-operation as to the service design this will not preclude competition between Us in respect of service provision. 7.4 The Primary Medical Services Providers must not do anything, or refrain or delay from doing anything. that would put the ICP in breach of the ICP Contract and the ICP must not do anything, or refrain or delay from doing anything, that would put the Primary Medical Services Providers in breach of their Services Contracts.
