Investor Address. If Investor is an individual, then Investor resides in the state or province identified in the address of Investor set forth on the signature page hereto. If Investor is a partnership, corporation, limited liability company or other entity, then the office or offices of Investor in which its investment decision was made is located at the address or addresses of Investor set forth on the signature page hereto.
