Issue and change tracking. TVS will report issues through the regular reporting mechanism (e.g. weekly reports). However, TVS also proposes that they also be reported through an online issue tracking systems so both sides can track them and they can be eventually closed through testing of the solutions. TVS understands that any design issues reported in phase N will be fixed in phase N+1 so each phase is stable wrt requirements.
