LETTER FOR QUALIFICATION. Bidders are to submit the Letter for Qualification in the Response Attachments tab in Ebid to including the following: 1. General Information – TAB 1 a. Firm/Individual name, street address, telephone number and email address. b. Years established and all former names. c. Types of services particularly qualified to perform. d. Name of principals and all states where registered. e. Names of key personnel to be used for the proposed work, experience of each and length of service. f. Average number of staff employed. g. List of completed projects (minimum of 3) from the office which will be performing for this contract including the following: i. The project’s original cost and the final cost of the project. ii. The initial proposed completion time and the actual completion time. h. A minimum of three (3) references that can be contacted by the Owners including contact names, email addresses and telephone numbers. i. Describe how you would effectively work with the Owners. j. Do you have a list of preferred subcontractors that you typically use? 1. If yes, please list.
