Those risks include, but are not limited to, those that are discussed above. I acknowledge that those risks include virtually any kind of permanent injury and even death. I further recognize that my access to medical care while at sea will be very limited, and that at times it may be impossible to obtain timely and adequate medical care for any injury or illness. I have decided to participate in this activity in full awareness of the risks described above. I further understand, acknowledge and agree that SEA and its captains have complete discretion to determine who they will take to sea, and, if any problem arises, who may continue to participate in the voyage. Liability Waiver and Release. It is understood and agreed that Skate Kingston shall NOT be responsible or held liable for any damages, injury, or loss of property to any member, guest or visitor to Skate Kingston regardless of the reason or nature of such damage, loss or injury. Every member, guest or visitor who participates in Skate Kingston on-ice or off-ice activities shall use the Club facilities at his or her own risk. Skaters that are registered 3 or 4 days per week must provide $500 in post-dated cheques (2 cheques each $250). For families with 2 or more skaters in STARSkate the volunteer amount is based on total number of days skating for the family. For example, a family with 2 skaters each skating 2 days per week, that family would have the commitment of $500. Cheques are payable to Skate Kingston, with 1 dated for Dec 1/17 and the other for March 1/18. If only writing one cheque please make it for March 1/18. I understand that these cheques will be “held in trust” until I have fulfilled all of my volunteer commitment. Once I have fulfilled all of my volunteer hours, these cheques will be returned to me.
