LICENSEE Contact Information. LICENSEE must provide to the REGENTS the completed LICENSEE contact information form attached hereto as APPENDIX B concurrent to execution of this AGREEMENT and incorporated herein by this reference, showing the contacts responsible for (i) Patent Prosecution, (ii) Progress Reports, (iii) Royalty Reports, and (iv) Invoices.
