Limitation on Provider Coverage. The provider must be licensed, or otherwise authorized by law, in the jurisdiction where the services are rendered. In addition, to be covered, the services must be within the lawful scope of the services for which that provider is licensed or otherwise authorized by law. Coverage does not include services rendered to Members by: SAMPLE A. The Member him/herself, or by the Member’s Spouse, mother, father, daughter, son, brother, or sister; or, B. Anyone who resides in the Member’s home.
