LOAD TICKETS. The Purchaser will be issued load ticket books with consecutively numbered tickets for uniquely identifying truckloads of logs. A load ticket shall be attached to the driver’s side bunk log of each product load prior to hauling, with the Scaler’s copy given to the State from the delivery point. The Purchaser shall complete each ticket with the sale information required by the State. The Purchaser shall provide to the Forest Officer a complete list of destinations to which loads will be delivered. The State, at its discretion, may require a separate ticket book for each destination. Failure to comply with this provision is a contract violation.
LOAD TICKETS. In conjunction with the debris monitoring contractor, load tickets will be used for recording volumes of debris removal. The Contractor shall provide an automated debris management ticketing and accounting process for debris management projects. Each ticket will be a five-part carbon copy ticket, and or electronic duplicates which shall contain the following information: • Ticket Number • Contractor’s Name • Crew Number • Truck Number • Date • Debris Removal (Pickup) Location • Debris Removal (Pickup) Location Departure Time • TDSR or Disposal Site Location • TDSR or Disposal Arrival Time • Debris Classification • Debris Quantity • Signed by a County representative Debris quantity and load tickets will be determined by contracted debris monitoring personnel or the County at the TDSR and/or disposal site. Based on predetermined truck bed measurements, trucks with less than full capacity will be adjusted downward by visual inspections. Truck bed measurements will not be adjusted upward. Load tickets will be issued by contracted debris monitoring personnel or the County and issued to vehicle operators upon completion of collection at the collection site. Five copies of load tickets will be issued to the County (1) and Contractor (4) to remain with Contractor’s records and TDSR or disposal sites.
LOAD TICKETS. The following Load Ticket is meant to be an example only for the purpose of this RFP. The actual Load Tickets used may vary slightly but will have representative information. Each load of Eligible Debris shall be tracked using a multi-page load ticket similar to the one shown below. If an automated debris management system is utilized, it must be a secure platform designed to prevent fraudulent activity and must contain all necessary information to ensure both FEMA and FHWA reimbursement requirements are met. 3.2. LOAD TICKETS. 3.1 While it is preferred that an e-ticket system will be utilized for monitoring, should a manual system be used, load tickets shall be used for recording the cubic yard volume of debris removed for disposal. In the event that an e-ticket system is not utilized, the following process will be used regarding load tickets. A copy of the load ticket to be used by the Contractor shall be submitted for City approval prior to beginning work. The Contractor shall provide all load tickets to the City. The load ticket numbers shall be sequentially numbered. The load tickets shall be a minimum of four-parts. A sample load ticket is included as an attachment.
3.2 The e-ticket system, or if manual tickets are used, shall contain the following information: • Ticket Number • Contract Number • Contractor Name • Date • Truck or Roll-off Number • Truck Capacity • Point of Debris Collection • Loading Departure Time • Dump Arrival Time • Percent of Load • Actual Debris Volume • Debris Eligibility (Y/N) • Type of Debris
3.3 In the event manual tickets are used, a minimum of four-part load ticket will be issued by a City monitor prior to transport of the debris from the loading site. The entire four-part load ticket is given to the vehicle operator. Upon arrival at the dumpsite, the vehicle operator will give the entire four-part load ticket to the City monitor. The City monitor will verify the hauler and equipment and establish a percent of truck capacity of the eligible cubic yardage of debris load. After documenting percentage to the nearest 5%, the City monitor will calculate the actual cubic yardage of the load. The actual cubic yard will be recorded on the load ticket by the City monitor to the nearest cubic yard. The City monitor will document the data on the load ticket. The City monitor will give one copy to the vehicle operator. One copy is then given to the Contractor. The original will be kept by the City and the fourth will be provided to any other entity that may need a copy. The load tickets shall be submitted with the daily operational report.
LOAD TICKETS. The Contractor will be issued load ticket books with consecutively numbered tickets for uniquely identifying truckloads of logs. LOAD TICKETS. Vendor shall provide tickets to drivers on a daily basis at the time of disposal. Tickets shall be legible and include the City truck number, date, time and number of tons (OR) total cubic yards of the load. Ticket numbers shall not be duplicated. Disposal will be charged on a per ton or per cubic yard basis for yard waste, and wood waste. Invoicing is to be completed and sent to the City on a monthly basis. Invoices shall include ticket dates, ticket numbers, number of tons (OR) cubic yard quantity of each load and the charge of each transaction.
LOAD TICKETS. A five-part document, in a form approved by the County Contract Administrator, which tracks Disaster Debris from the original collection point to the TDMS.
LOAD TICKETS. 1. Serialized, four-part Load Tickets or electronic version, shown within Attachment A, attached hereto and incorporated herein, shall be used for recording cubic yards/tons of Eligible Debris removed from Work Zones and for recording cubic yards/tons of debris removed from the DMS for final disposition at an approved landfill or recycling facility.
2. All tickets shall be distributed in numerical order and the numbering system shall be unique to the City of Pompano Beach. Numbers shall be recorded on a Load Ticket Log, attached hereto and incorporated herein within Attachment A, by the CA or authorized designee. No Load Tickets shall be unaccounted for. If a Load Ticket is voided for any reason, at least one copy of the ticket must be retained by both the CONTRACTOR and the CITY for accounting purposes.
3. Each Load Ticket shall contain the following information: -Preprinted ticket number -Assigned vehicle/equipment number -VIN number (if, requested) -Vehicle/equipment driver’s name -Contract number -CONTRACTOR name -Date -Loading time -Dumping time -Maximum capacity in cubic yards -Load size, either in tons or cubic yards -Debris classification -Assigned Work Zone -Dumpsite location (DMS/final disposition site) -Work zone monitor’s signature -Dumping site supervisor’s signature (DMS/final disposition site)
4. The Load Tickets for debris hauling shall be completed upon arrival at the DMS and a new ticket initiated upon the departure of debris hauling trucks at the inspection tower(s) located at the entry/exit point for each DMS, to be completed at the final disposition site.

The COR will keep one copy of the ticket, and give three copies to the vehicle operator. Upon arrival at the dumpsite, the vehicle operator will give the three copies to the COR at the dumpsite, the COR will validate, retain one copy and give two copies to driver for the Contractor’s records, (one copy for the sub-contractor and one copy for the prime contractor). Load tickets will be issued by a COR prior to departure from the loading site. The COR will keep one copy of the ticket, and give two copies to the vehicle operator for the Contractor’s records. Load tickets will be issued by a COR to a vehicle operator upon arrival at the dumpsite. The COR will keep one copy of the ticket, and give two copies to the vehicle operator for the Contractor’s records.
