LONG-TERM CARE COVERAGE. Policies of this category are designed to provide coverage for one or more necessary or medically necessary diagnostic, preventive, therapeutic, rehabilitative, maintenance, or personal care services, provided in a setting other than an acute care unit of a hospital, such as in a nursing facility, in the community or in the home. The Policy provides coverage in the form of an expense incurred benefit for covered qualified long-term care expenses, subject to benefit eligibility, policy limitations, elimination periods, and daily and lifetime policy maximums.
LONG-TERM CARE COVERAGE. Agreements of this category are designed to provide coverage for one or more necessary diagnostic, preventive, therapeutic, rehabilitative, maintenance, or personal care services, provided in a setting other than an acute care unit of a hospital, such as in a nursing home, in the community, or in the home. The Agreement provides coverage for qualified long-term care services in the form of an acceleration of your death benefit and is subject to the elimination period, limitations, and exclusions described in the Agreement.
LONG-TERM CARE COVERAGE. In addition to the health benefit coverage as outlined above, the Board shall provide UNUM Long Term Care Indemnity Base Plan (Plan 1). Each eligible employee may choose to add family members and/or increase this coverage from the base plan by utilizing the Buy-up Options as outlined by UNUM, at the employee’s expense. The Base Plan coverage is $3000/month with Facility Benefit Duration of 3 years and a lifetime maximum benefit of $108,000. Any other bargaining unit member may obtain this coverage for themselves or add family members, at their expense, utilizing the UNUM group premium rate.
LONG-TERM CARE COVERAGE. The Employer and the Union agree the issue of allowing employees to participate in a voluntary long- term care coverage program shall be forwarded to the Labor Management Committee on Insurance.
