Medical Lockbox Services a. The Bank offers a specific service known as “Medical Lockbox” pursuant to which the Bank provides a healthcare remittance service processing payments and documents related to healthcare services. When you utilize the Bank’s Health Care Remittance Service you represent and warrant that by entering into and engaging in the activities contemplated by this Agreement, including the Documentation, you are not in violation of and will not be violating any agreements with third parties, including but not limited to, contracts with payers and/or consents and directives of patients, or any federal or state laws. You agree to comply with all applicable federal and state law regarding the privacy of health information, including but not limited to, the provisions of the Health Insurance Portability and Accountability Act of 1996 and the regulations promulgated thereunder, as amended from time to time (“HIPAA”). You understand and agree that, as necessary for the delivery of the Services, we and our contractors and agents may have access to and use of Protected Health Information, as that term is defined in HIPAA, which is transmitted or otherwise made available to us. You represent and warrant that you have, and will continue to have, the authority to allow us and our contractors and agents to have access to and use of such Protected Health Information.
b. In order to use the HealthCare Remittance Service a medical provider will send their customer a paper ▇▇▇▇ (invoice and statement with information in a scanable line) for an outstanding balance. Upon receipt of the paper ▇▇▇▇ at the Bank’s lockbox facility, the envelope is opened, the payment processed and the statement and any other documents are scanned to create an electronic file in the X12 format established by The Accredited Standards Committee (“ASC”), chartered by the American National Standards Institute. Known as the ASC X12 835 (Healthcare Claim Payment / Remittance Advice) or “835” file.
