Member Information a. ODM, or its designee, will provide membership notices, informational materials, and instructional materials to members and eligible individuals in a manner and format that may be easily understood. At least annually, ODM or its designee will provide current MCP members with an open enrollment notice which describes the managed care program and includes information on the MCP options in the service area and other information regarding the managed care program as specified in 42 CFR 438.10.
b. ODM will notify members or ask the MCP to notify members about significant changes affecting contractual requirements, member services or access to providers.
c. If the MCP elects not to provide, reimburse, or cover a counseling service or referral service due to an objection to the service on moral or religious grounds, ODM will provide coverage and reimbursement for these services for the MCP’s members.
d. As applicable, ODM will provide information to MCP members on what services the MCP will not cover and how and where the MCP’s members may obtain these services.

c. If the MCOP elects not to provide, reimburse, or cover a counseling service or referral service due to an objection to the service on moral or religious grounds, ODM will provide coverage and reimbursement for these services for the MCOP’s members.
d. As applicable, ODM will provide information to the MCOP’s members on what services the MCOP will not cover and how and where the MCOP’s members may obtain these services.
Member Information. (a) In addition to the other rights specifically set forth in this Agreement, each member is entitled to the information to which that Member is entitled to have access pursuant to the Act, under the circumstances therein stated.
(b) The Members acknowledge that, from time to time, they may receive information from or concerning the Company in the nature of trade secrets, the release of which may damage the Company or Persons with which it does business. Each Member shall hold in strict confidence any trade secret information that it receives concerning the Company that is identified as being confidential (and if that information is provided in writing, that is so marked) and may not disclose it to any Person other than another member or Governor, except for disclosures (i) compelled by law (but the Member must notify a Governor promptly of any request for that information, before disclosing it, if legal and practicable); (ii) to Persons to whom that Member’s Interest may be transferred as permitted by this Agreement, but only if the recipients have agreed to be bound by the provisions of this Section 7.7, or (iii) of information that the Member also has received from a source independent of the Company and the Member reasonably believes that source obtained the information without breach of any obligation of confidentiality. The Members acknowledge that breach of the provisions of this Section 7.7 may cause irreparable injury to the Company for which monetary damages are inadequate, difficult to compute, or both. Accordingly, the Members agree that the provisions of this Section 7.7 may be enforced by specific performance.
Member Information. Within five (5) days after receipt of a request from the Company, the Investor agrees to provide such information with respect to its status as a member (or potential member) of the Company and to execute and deliver such documents as may reasonably be necessary to comply with any and all laws and regulations to which the Company is or may become subject, including, without limitation, the need to determine the accredited investor status of the Company’s members. If the Investor transfers any Units, it will require the transferee of such Units to agree to provide such information to the Company requires as a condition of such transfer.
Member Information. In performing its obligations under this Agreement, neither Member shall be obligated to disclose any Member Information. If a Member elects to disclose Member Information in performing its obligations under this Agreement, such Member Information, together with all improvements, enhancements, refinements and incremental additions to such Member Information that are developed, conceived, originated or obtained by either Member in performing its obligation under this Agreement (“Enhancements”), shall be owned exclusively by the Member that originally developed, conceived, originated or obtained such Member Information. Each Member may use and enjoy the benefits of such Member Information and Enhancements in the conduct of the Business hereunder, but the Member that did not originally develop, conceive, originate or obtain such Member Information may not use such Member Information and Enhancements for any other purpose. Except as provided in Section 13.4, or with the prior written consent of the other Member, which consent may be withheld in such Member’s sole discretion, each Member shall keep confidential and not disclose to any third party or the public any portion of Member Information and Enhancements owned by the other Member that constitutes Confidential Information.
Member Information. On or about November 20, March 20, and July 20 of each year, the Employer will make available to the CASBU Officer, an up-to-date Membership list which will include the name, the Faculty and Department, and the total amount of dues to be deducted of each Member.
Member Information. Within five days after receipt of a request from the Company, the Subscriber hereby agrees to provide such information with respect to its status as a member (or potential member) and to execute and deliver such documents as may reasonably be necessary to comply with any and all laws and regulations to which the Company is or may become subject. Subscriber further agrees that in the event it transfers any Securities, it will require the transferee of such Securities to agree to provide such information to the Company as a condition of such transfer.
Member Information. The MCO must comply with applicable federal and state laws regarding persons with limited English proficiency (LEP) and persons with disabilities, including Title VI of the Civil Rights Act of 1964, Titles II and III of the Americans with Disabilities Act, Section 504 of the Rehabilitation Act of 1973, and section 1557 of the Patient Protection and Affordable Care Act. The MCO must develop and implement a written plan or policy governing accessibility and accommodations for persons with LEP and persons with disabilities. The plan shall require training of pertinent staff on the process. The plan must be made available for review by ODM at ODM’s request. The MCO must provide written notice of nondiscrimination (i.e., that the MCO may not discriminate on the basis of race, color, religion, gender, gender identity, sexual orientation, age, disability, national origin, military status, ancestry, genetic information, health status, or need for health services in the receipt of health services) to members. The MCO must provide written information to members describing how to request language or disability accommodations and how to file a grievance with both the MCO and the HHS Office of Civil Rights. The MCO must comply with the following information requirements for eligible individuals and members, in accordance with applicable federal and state law, including 42 CFR §438.10 and OAC rules 5160-26-03.1, 5160-26-05, and 5160-26-05.1:
Member Information. For the purposes of this Agreement, “Member Information” shall mean any non-public, personally identifiable information about a Member, including any combination of a Member’s name plus any of his or her social security number, driver’s license or other identification number or credit or debit card number, or other account number utilized by a Service Provider, revealed by or through a Disclosing Party to a Receiving Party.
Member Information a) The Employer agrees to provide the SEIU-West with a list of all employees and their last known address on a quarterly basis with the first (1st) of such lists to be forwarded by March 1st of each year.
b) It shall be the responsibility of the employee to notify the Employer of any change of name, marital status, place of residence or telephone number. The Employer shall forward a copy of such notification of changes to name, place of residence or telephone number to the SEIU-West within thirty (30) days of having received same.
