Monthly Service Fees. The Member must pay the following monthly service fee based upon his/her age: 4.1.1 Children under the age of 18. If the Member is under the age of 18 the Member’s monthly service fee is $57.00 (before discounts). 4.1.2 Between the age of 19-50. If the Member is between the ages of 19-50 the Member’s monthly service fee is $77.00 (before discounts).
