Upon a bona fide application for underground service from an overhead system to an Applicant's premises, an underground service connection will be provided by the Utility in accordance with the following provisions: (A) the Applicant shall 1. provide and maintain, at his expense, any required transformer vault, pull box, or other duct termination facilities, 2. perform the necessary trenching, backfill, and paving on his property and on the Utility easement or right-of-way to the pole,
