Notice of Adverse Actions. Provider shall promptly notify BCBSM of any action, determination, or circumstance involving Provider, or an officer, director, owner or principal of Provider, which affects or may affect the provision of Covered Services. Such circumstances shall include, without limitation, the following: a. Plea of guilty or nolo contendere or conviction or placement in a diversion program for any crimes related to the payment or provision of health care; b. Censure, reprimand, resolution, suspension, exclusion, revocation, or reduction to probationary status of Provider's license or Medicare certification; c. Exclusion or debarment from any state or federal program.
