Notice to Relocate. If either party desires to relocate, the parties agree to comply with SDCL 25-4A-17 and, if applicable, SDCL 25-4A-18.
Notice to Relocate. Primary Care Provider is currently providing services to Members at the location(s) of service listed on the Credentialing application. If Primary Care Provider closes, changes, or adds new locations, Primary Care Provider shall notify RHS immediately.
