OBLIGATIONS OF THE ATHLETE. The ATHLETE shall: a. Maintain status as a member in good standing of FHC and as per policies set by FHC, pay any and all membership dues, insurance fees, and national team levies as approved and assessed by the National Team programs and FHC from time to time; b. Under the supervision of the National Coach or his/her designate, actively participate in all training programs, competitive activities, evaluation activities, and reporting procedures approved by the National Coach or by FHC for the National Teams Program; c. To play field hockey outside of the FHC system the ATHLETE must obtain clearance from the National Coach; d. Demonstrate commitment to high performance field hockey by: i. Developing and maintaining a high level of fitness; ii. Following an individual training and competitive program approved by the National Coach; iii. Maintaining regular contact with the National Coach or with his/her designate, iv. Maintaining training logs and reporting as requested to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the National Coach or his/her designate; and vi. Living in an environment that is conducive to high performance athletic achievement and avoiding activities that put the ATHLETE’S ability to perform at risk; e. Within one week (7 days), notify the National Coach or his/her designate verbally of any injury or other legitimate reason that might prevent the ATHLETE from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosis, the ATHLETE shall supply the National Coach with a certificate from a medical doctor describing the nature of the injury. Athletes approved for out of country training must contact the National Coaches directly if they foresee any problems meeting the above timelines around injury reporting; f. Provide pertinent medical information to FHC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs; g. Wear and use approved national team uniforms at all national team activities including training, competition, competition-day warm-ups, opening and closing ceremonies, post-competition ceremonies, press conferences, photo sessions, promotional events and other public appearances, at all times and in the proper manner as directed by FHC. ATHLETES wishing to be selected for Major Games, agree to sign other agency agreements related to sponsorship and promotion. The athlete player representative will be consulted as part of any major review of the national team training and competition uniform. Decisions on training and competition uniforms will be the responsibility of FHC. All decisions will be made within the guidelines set out by the International Hockey Federation (FIH) and/or Major Games sponsorship and promotions agreements as relevant; h. Participate in reasonable educational or non-commercial, promotional activities as may be requested by FHC or the Government of Canada, provided the ATHLETE shall be compensated for out-of-pocket expenses and such activities will not exceed the equivalent of two days per year. FHC usually makes such requests for participation and arranges the activities. Unless supplementary compensation is arranged, these activities do not normally involve more than three working days per athlete per year. This is in addition to commercial appearances that the ATHLETE may make on behalf of FHC as described in Section 3(a)(ii) of this Agreement; i. Supply FHC with biographical information as requested, and notify FHC and the Athlete Assistance Program immediately of any change of address; j. As a member of FHC and of the National Teams Program, behave in a lawful, courteous and respectful manner and adhere at all times to FHC’s policies and rules relating to Athlete conduct in effect from time to time; k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26, 2019), and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change; l. Execute any further documents that may be required by FHC to give effect to the undertakings set out in this Agreement; m. Indemnify and hold harmless FHC from any losses that FHC may incur as a result of the ATHLETE’S failure to satisfy any provision(s) of this Agreement. This indemnification shall survive termination of this Agreement; n. During or at National Team training camps and competitions, avoid alcoholic consumption to a level that would reasonably be expected to impair the Athlete’s ability to speak, walk, or drive, or cause the Athlete to behave in a disruptive manner; o. Avoid the use of banned substances that contravene the rules of the International Olympic Committee (IOC), the rules of the International Federation (FIH) and the Canadian Policy on Doping in Sport; p. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so; q. Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices; r. Complete the CCES on line anti-doping education program’s 2 courses “True Sport Clean” and “Sport Canada - Athlete Assistance Program” or reviews if courses have been completed previously at the beginning of each new carding cycle. Failure to do so will result in AAP payments being kept on hold until requirement is met. s. Avoid participating in any competitions where federal government sport policy has determined that such participation is not permitted; t. Adhere to and comply with FHC’s appeal procedure; u. Actively participate in all AAP evaluation activities. ATHLETES will co-operate fully in any evaluation of the AAP that may be conducted by the Minister or anyone authorized to act on the Minister’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation; v. Actively participate in all FHC evaluation activities. ATHLETES will co-operate in any evaluation of FHC, in particular the High Performance Programs that may be conducted by the Board, National Team working groups or anyone authorized to act on FHC’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation; w. Avoid any action or conduct that would reasonably be expected to significantly disrupt or interfere with a competition or the preparation of any Athlete for a competition.

f. Provide pertinent medical information to FHC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. j. As a member of FHC and of the National Teams Program, behave in a lawful, courteous and respectful manner and adhere at all times to FHC’s policies and rules relating to Athlete conduct in effect from time to time;
k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26Policies, 2019)Procedures and Guidelines, and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change;
l. Execute any further documents that may be required by FHC to give effect to the undertakings set out in this Agreement;
m. Indemnify and hold harmless FHC from any losses that FHC may incur as a result of the ATHLETE’S failure to satisfy any provision(s) of this Agreement. This indemnification shall survive termination of this Agreement;

p. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so;
q. Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices;
r. Respect and abide by the Canadian Anti-Doping Program (CADP) as administered by the Canadian Centre for Ethics in Sport (CCES);
i. Specifically agree that as a member of the National Athlete Pool (NAP) in my sport I am subject to the Canadian Anti-Doping Program (CADP) and accordingly shall be bound by all the anti-doping rules and responsibilities contained in the CADP.
ii. Agree that I have been educated regarding the anti-doping rules and violations contained in the CADP.
iii. Acknowledge that information, including personal information about me, can be shared between anti-doping organizations for anti-doping purposes and such information will be used only in a fashion that is fully consistent with the limitations and restrictions contained in the World Anti-Doping Agency’s International Standard for the Protection of Privacy and Personal Information.
iv. With the understanding that any disclosure is for the sole purpose of assisting the CCES in enforcement of the CADP, I consent to having police and law enforcement agencies, border services agencies, Sport Organizations of which I am a member and sporting clubs and athletic associations to which I belong, in Canada and elsewhere, disclose to the CCES information in their possession relating to me that is directly relevant to potential anti-doping rule violations contained in the CADP that may be asserted against me.
s. Complete the CCES on line anti-doping education program’s 2 courses “courses, True Sport Clean” Clean 101 and “Sport Canada - Athlete Assistance Program” or reviews if courses have been completed previously , at the beginning of each new carding cycle. Failure to do so will result in AAP payments being kept on hold until requirement is metcycle and at times thereafter as required by Sport Canada.
s. t. Avoid participating in any competitions where federal government sport policy has determined that such participation is not permitted;
t. u. Adhere to and comply with FHC’s appeal procedure;
u. v. Actively participate in all AAP evaluation activities. ATHLETES will co-operate fully in any evaluation of the AAP that may be conducted by the Minister or anyone authorized to act on the Minister’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation;
v. w. Actively participate in all FHC evaluation activities. ATHLETES will co-operate in any evaluation of FHC, in particular the High Performance Programs that may be conducted by the Board, National Team working groups or anyone authorized to act on FHC’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation;.
w. x. Avoid living in an environment not conducive to high-performance achievements or taking any deliberate action that puts his or her ability to perform at risk or limits performance.
y. Avoid any action or conduct that would reasonably be expected to significantly disrupt or interfere with a competition or the preparation of any Athlete for a competition.

ii. Following an individual training and competitive program approved by the NextGen Director and Junior National Coach;
iii. Maintaining regular contact with the National Coach or with his/her designate,
iv. Maintaining training logs and reporting as requested to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the Junior National Coach or his/her designate; and
vi. Living in an environment that is conducive to high performance athletic achievement and avoiding activities that put the ATHLETE’S ability to perform at risk;
e. Within one week (7 days), notify the Junior National Coach or his/her designate verbally of any injury or other legitimate reason that might prevent the ATHLETE from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosis, the ATHLETE shall supply the Junior National Coach with a certificate from a medical doctor describing the nature of the injury. Athletes approved for out of country training must contact the National Coaches directly if they foresee any problems meeting the above timelines around injury reporting;
f. Provide pertinent medical information to FHC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. For all NextGen, Youth or at Junior National Team training camps and competitionsAthletes, avoid consumption of alcoholic consumption beverages during, before, after or while travel for, team projects, events or competitions regardless of if they are of government legal age to a level that would reasonably be expected to impair the Athlete’s ability to speakconsume alcohol, walk, or drive, or cause the Athlete to behave in a disruptive manner;is strictly prohibited.
o. m. Avoid the use of banned substances that contravene the rules of the International Olympic Committee (IOC), the rules of the International Federation (FIH) and the Canadian Policy on Doping in Sport;
p. n. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so;

OBLIGATIONS OF THE ATHLETE. The ATHLETE Athlete shall:

b. b) Where CKC is to reimburse the Athlete for expenses, promptly deliver to CKC all receipts and an expense claim form to the National Office. Where expense claims are not accompanied by receipts, payment on such claims may not be made;
c) Under the supervision of the National Team Head Coach or his/her designateother authorized coach, actively participate in all training programs, competitive activities, evaluation activities, activities and reporting procedures approved by the National Coach or by FHC Slalom High Performance Committee for the National Teams ProgramTeam;
c. To play field hockey outside of d) Compete in the FHC system National Slalom Championships, the ATHLETE must obtain clearance Trials and at least 2 regional races, unless excused in advance by the Slalom High Performance Committee, or National Team Head Coach;
e) Meet the training standards and follow the training program established by National Team Head Coach, and communicate regularly on progress with National Team Head Coach or his or her designate.. Failure to meet these standards or follow this program may result in removal from the Athlete Assistance Program where applicable and/or removal from the National CoachTeam;
d. Demonstrate commitment to high performance field hockey by:
i. Developing and maintaining a high level of fitness;
ii. Following an individual training and competitive program approved by the National Coach;
iii. Maintaining regular contact with the National Coach or with his/her designate,
iv. Maintaining training logs and reporting as requested f) Submit to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the National Team Head Coach or his/his or her designate; and
vi. Living in an environment that is conducive to high performance athletic achievement designate yearly training and avoiding activities that put the ATHLETE’S ability to perform at riskcompetition plans, and bi-weekly training logs;
e. Within one week (7 days)g) At the earliest possible date, notify the National Team Head Coach or his/her designate verbally and the CKC National Office in writing of any injury or other legitimate reason that might prevent the ATHLETE Athlete from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosisinjury, the ATHLETE Athlete shall supply the National Team Head Coach and with a certificate from a medical doctor describing the nature of the injury within one week of the diagnosis of the injury, or immediately upon diagnosis if there is a scheduled event within the one-week period. Athletes approved for out CKC reserves the right to seek a second opinion from a certified medical practitioner of country training must contact its choice and the National Coaches directly if they foresee Athlete shall co-operate with any problems meeting the above timelines around injury reportingsuch request;
f. h) Provide pertinent medical information to FHCCKC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. i) Wear and use approved national team National Team competition uniforms at all national team National Team activities including training, competition, competition-day warm-upscompetition events, opening and closing ceremonies, post-competition ceremonies, medal ceremonies, press conferences, photo sessions, promotional events and other public appearances, at all times and in the proper manner as directed by FHC. ATHLETES wishing to be selected for Major Games, agree to sign other agency agreements related to sponsorship and promotion. The athlete player representative will be consulted as part of any major review of the national team training and competition uniform. Decisions on training and competition uniforms will be the responsibility of FHC. All decisions will be made within the guidelines set out by the International Hockey Federation (FIH) and/or Major Games sponsorship and promotions agreements as relevantCKC;
h. j) Participate in reasonable educational or non-commercial, promotional activities as may be requested by FHC CKC or the Government of Canada, provided the ATHLETE Athlete shall be compensated for out-of-pocket expenses and such activities will not exceed the equivalent of two days per year. FHC usually makes such requests for participation and arranges the activities. Unless supplementary compensation is arranged, these activities do not normally involve more than three working days per athlete per year. This is in addition to commercial appearances that the ATHLETE may make on behalf of FHC as described in Section 3(a)(ii) of this Agreement;
i. Supply FHC with biographical information as requested, k) Assign and notify FHC and does hereby assign to CKC the Athlete Assistance Program immediately of any change of address;
j. As a member of FHC and of the National Teams Program, behave in a lawful, courteous and respectful manner and adhere at all times exclusive worldwide right to FHC’s policies and rules relating to Athlete conduct in effect from time to time;
k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26, 2019), and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change;
l. Execute any further documents that may be required by FHC to give effect to the undertakings set out in this Agreement;
m. Indemnify and hold harmless FHC from any losses that FHC may incur as a result of the ATHLETE’S failure to satisfy any provision(s) of this Agreement. This indemnification shall survive termination of this Agreement;
n. During or at National Team training camps and competitions, avoid alcoholic consumption to a level that would reasonably be expected to impair use the Athlete’s ability image, name, nickname or other likeness for the promotion of competitive canoeing competitions organized by CKC or in which CKC is a participant. Such assignment shall be in perpetuity. After the athlete has retired from active international competition, use of the image is subject to speakathlete and agent approval, walk, or drive, or cause the Athlete such approval not to behave in a disruptive mannerbe unreasonably withheld;
o. l) Warrant that he/she has not and will not enter into any promotional or commercial sponsorship or endorsement within the tobacco industry involving the use of the Athlete’s image, name, nickname or other likeness;
m) Avoid the use of banned substances that contravene the rules of the International Olympic Committee (IOC), the rules of the International Federation (FIH) Canoe Federation, the rules of CanoeKayak Canada and the Canadian Policy on Penalties for Doping in Sport;
p. n) Submit, without prior warningnotice, to unannounced doping-control tests in addition to other prior-prior- notice tests and submit at other times to doping-control testing when requested by FHCCanoe Kayak Canada, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so;
q. o) Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices;
r. v) Comply with the by-laws, rules, regulations and policies of CKC, as adopted and amended from time to act on FHC’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct time;

iv. Maintaining training logs and reporting as requested to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the National Coach or his/her designate; , and
vi. Living in an environment that is conducive to high performance athletic achievement and avoiding activities that put the ATHLETE’S ability to perform at risk;.

Within one week (7 days), notify the National Coach or his/her designate verbally of any injury or other legitimate reason that might prevent the ATHLETE from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosis, the ATHLETE shall supply the National Coach with a certificate from a medical doctor describing the nature of the injury. Athletes approved for out of country training Athlete’s Initials must contact the National Coaches directly if they foresee any problems meeting the above timelines around injury reporting;.
f. Provide pertinent medical information to FHC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. Wear and use approved national team uniforms at all national team activities including training, competition, competition-day warm-ups, opening and closing ceremonies, post-competition ceremonies, press conferences, photo sessions, promotional events and other public appearances, at all times and in the proper manner as directed by FHC. ATHLETES wishing to be selected for Major Games, agree to sign other agency agreements related to sponsorship and promotion. The athlete player representative will be consulted as part of any major review of the national team training and competition uniform. Decisions on training and competition uniforms will be the responsibility of FHC. All decisions will be made within the guidelines set out by the International Hockey Federation (FIH) and/or Major Games sponsorship and promotions agreements as relevant;
h. Participate in reasonable educational or non-commercial, promotional activities as may be requested by FHC or the Government of Canada, provided the ATHLETE shall be compensated for out-of-pocket expenses and such activities will not exceed the equivalent of two days per year. FHC usually makes such requests for participation and arranges the activities. Unless supplementary compensation is arranged, these activities do not normally involve more than three two working days per athlete per year. This is in addition to commercial appearances that the ATHLETE may make on behalf of FHC as described in Section 3(a)(ii3 (a) (ii) of this Agreement;
i. Supply FHC with biographical information as requested, and notify FHC and the Athlete Assistance Program immediately of any change of address;
j. As a member of FHC and of the National Teams Program, behave in a lawful, courteous and respectful manner and adhere at all times to FHC’s policies and rules relating to Athlete conduct in effect from time to time;
k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26Policies, 2019)Procedures and Guidelines, and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change;

p. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so;
q. Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices;
r. Respect and abide by the Canadian Anti-Doping Program (CADP) as administered by the Canadian Centre for Ethics in Sport (CCES);
i. Specifically agree that as a member of the National Athlete Pool (NAP) in my sport I am subject to the Canadian Anti-Doping Program (CADP) and accordingly shall be bound by all the anti-doping rules and responsibilities contained in the CADP.
ii. Agree that I have been educated regarding the anti-doping rules and violations contained in the CADP.
iii. Acknowledge that information, including personal information about me, can be shared between anti-doping organizations for anti-doping purposes and such information will be used only in a fashion that is fully consistent with the limitations and restrictions contained in the World Anti-Doping Agency’s International Standard for the Protection of Privacy and Personal Information.
iv. f. h. Provide pertinent medical information to FHC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. Wear i. Subject to any contractual obligations of FHC to a clothing supplier as FHC may advise the ATHLETE, whenever reasonably practicable wear and use approved current national team uniforms at all national team activities including formal pre - event training, competition, competition-day warm-ups, opening and closing ceremonies, post-post- competition ceremonies, press conferences, photo sessions, promotional events and other public appearances, at all times and in the proper manner as directed by FHC. ATHLETES wishing to be selected for Major Games, agree to sign other agency agreements related to sponsorship and promotion. The athlete player representative will be consulted as part of any major review of the national team training and competition uniform. Decisions on training and competition uniforms will be the responsibility of FHC. All decisions will be made within the guidelines set out by the International Hockey Federation (FIH) and/or Major Games sponsorship and promotions agreements as relevant;
h. Participate in reasonable educational or non-commercial, promotional activities as may be requested by FHC or the Government of Canada, provided the ATHLETE shall be compensated for out-of-pocket expenses and such activities will not exceed the equivalent of two days per year. FHC usually makes such requests for participation and arranges the activities. Unless supplementary compensation is arranged, these activities do not normally involve more than three working days per athlete per year. This is in addition to commercial appearances that the ATHLETE may make on behalf of FHC as described in Section 3(a)(ii) of this Agreement;
i. j. Supply FHC with biographical information as may be requested, and notify FHC and the Athlete Assistance Program immediately of any change of address;
j. k. As a member of FHC and of the National Teams Program, behave in a lawful, ,courteous and respectful manner and adhere at all times to FHC’s policies and rules relating to Athlete conduct in effect from time to time;
k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26, 2019), and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change;
l. Execute any further documents that may be reasonably required by FHC to give effect to the undertakings set out in this Agreement;
m. Indemnify and hold harmless FHC from any losses that FHC may incur as a result of the ATHLETE’S failure to satisfy any provision(s) of this Agreement. This indemnification shall survive termination of this Agreement;
n. During or at National Team training camps and competitions, avoid alcoholic consumption consuming intoxicants to a level that would reasonably be expected to impair the Athlete’s ability to speak, walk, or drive, or cause the Athlete to behave disruptively or in a disruptive mannermanner that brings disrepute to the Masters Program or FHC;
o. Avoid the use of banned substances that contravene the rules of the International Olympic Committee (IOC), the rules of the International Federation (FIH) FIH and the Canadian Policy on Doping in Sport;
p. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-prior- notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCES) or other authorities designated to do so;
q. Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices;
r. Complete the CCES on line anti-doping education program’s 2 courses “True Sport Clean” and “Sport Canada - Athlete Assistance Program” or reviews if courses have been completed previously at the beginning of each new carding cycle. Failure to do so will result in AAP payments being kept on hold until requirement is met.
s. Avoid participating in any competitions where federal government sport policy has determined that such participation is not permitted;
t. Adhere to and comply with FHC’s appeal procedure;
u. Actively participate in all AAP evaluation activities. ATHLETES will co-operate fully in any evaluation of the AAP that may be conducted by the Minister or anyone authorized to act on the Minister’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation;
v. s. Actively participate in all FHC evaluation activities. ATHLETES will co-operate in any evaluation of FHC, in particular the High Performance Programs that may be conducted by the Board, National Team Masters Program committees or sub-committees, working groups or anyone authorized to act on FHC’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluation;; and
w. t. Avoid any action or conduct that would reasonably be expected to significantly disrupt or interfere with a competition or the preparation of any Athlete for a competition.

OBLIGATIONS OF THE ATHLETE. The ATHLETE shall:
a. As a member of the Association, behave in a courteous and respectful manner and adhere at all times to the Association’s policies relating to conduct as amended from time to time, including but not limited to, the Discipline Policy, Appeals Policy, Alternative Dispute Resolution Policy, Harassment Policy, Gender Equity Policy, Social Media Policy, Confidentiality Policy and Conflict of Interest Policy. Copies of these policies are available on the Association’s web site or upon request from the national office;
b. Agree to provide their consent to be bound by the Universal Code of Conduct to Prevent and Address Maltreatment in Sport (the UCCMS), as amended from time to time, and to be subject to proceedings initiated related to potential violations of the UCCMS, including, without limitation, accepting the jurisdiction of the OSIC/Abuse-Free Sport if they are, at any point, designated as a UCCMS Participant by the Association;
c. Read and understand the Association’s Social Media Guidelines for Athletes and ask for clarification and additional guidance when necessary;
d. Maintain status as a member in good standing of FHC and as per policies set by FHC, pay any and all membership dues, insurance fees, and national team levies as approved and assessed by the National Team programs and FHC from time to time;
b. e. Under the supervision of the National Coach or his/her designate, actively participate in all training programs, competitive activities, evaluation activities, and reporting procedures approved by the National Coach or by FHC for the National Teams Program;
c. f. To play field hockey outside of the FHC system the ATHLETE must obtain clearance from the National Coach;
d. g. Demonstrate commitment to high performance field hockey by:
i. Developing and maintaining a high level of fitness;
ii. Following an individual training and competitive program approved by the National Coach;
iii. Maintaining regular contact with the National Coach or with his/her designate,
iv. Maintaining training logs and reporting as requested to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the National Coach or his/her designate; and
vi. Living in an environment that is conducive to high performance athletic achievement and avoiding activities that put the ATHLETE’S ability to perform at risk;
e. h. Within one week (7 days), notify the National Coach or his/her designate verbally of any injury or other legitimate reason that might prevent the ATHLETE from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosis, the ATHLETE shall supply the National Coach with a certificate from a medical doctor describing the nature of the injury. Athletes approved for out of country training must contact the National Coaches directly if they foresee any problems meeting the above timelines around injury reporting;

OBLIGATIONS OF THE ATHLETE. The ATHLETE Athlete shall:
a. a) Maintain his or her status as a member in good standing of FHC a member club of CKC and as per policies set by FHCa registered competitor of CKC, and promptly pay any and all membership duesdues and National Team levies;
b) Where CKC is to reimburse the Athlete for expenses, insurance fees, promptly deliver to CKC all receipts and national team levies as approved and assessed by an expense claim form to the National Team programs and FHC from time to timeOffice. Where expense claims are not accompanied by receipts, payment on such claims may not be made;
b. c) Under the supervision of the National Team Head Coach or his/her designateother authorized coach, actively participate in all training programs, competitive activities, evaluation activities, activities and reporting procedures approved by the National Coach or by FHC Slalom High Performance Committee for the National Teams ProgramTeam.
d) Compete in the National Slalom Championships and the Team Trials unless excused in advance by the Slalom High Performance Committe;
c. To play field hockey outside of e) Meet the FHC system training standards and follow the ATHLETE must obtain clearance training program established by National Team Head Coach, and communicate regularly on progress with National Team Head Coach or his or her designate. Failure to meet these standards or follow this program may result in removal from the Athlete Assistance Program where applicable and/or removal from the National CoachTeam;
d. Demonstrate commitment to high performance field hockey by:
i. Developing and maintaining a high level of fitness;
ii. Following an individual training and competitive program approved by the National Coach;
iii. Maintaining regular contact with the National Coach or with his/her designate,
iv. Maintaining training logs and reporting as requested f) Submit to the National Coach, v. Participating in athlete monitoring and sport science tests as requested by the National Team Head Coach or his/his or her designate; and
vi. Living in an environment that is conducive to high performance athletic achievement designate yearly training and avoiding activities that put the ATHLETE’S ability to perform at riskcompetition plans, and bi-weekly training logs;
e. Within one week (7 days)g) At the earliest possible date, notify the National Team Head Coach or his/her designate verbally and the CKC National Office in writing of any injury or other legitimate reason that might prevent the ATHLETE Athlete from fulfilling any obligations under this Agreement. In the event of injury and within one week of its diagnosisinjury, the ATHLETE Athlete shall supply the National Team Head Coach and with a certificate from a medical doctor describing the nature of the injury within one week of the diagnosis of the injury, or immediately upon diagnosis if there is a scheduled event within the one-week period. Athletes approved for out CKC reserves the right to seek a second opinion from a certified medical practitioner of country training must contact its choice and the National Coaches directly if they foresee Athlete shall co-operate with any problems meeting the above timelines around injury reportingsuch request;
f. h) Provide pertinent medical information to FHCCKC’s medical staff as requested, and consult with medical staff regarding the use of prescription and non-prescription drugs;
g. i) Wear and use approved national team National Team competition uniforms at all national team National Team activities including training, competition, competition-day warm-upscompetition events, opening and closing ceremonies, post-competition ceremonies, medal ceremonies, press conferences, photo sessions, promotional events and other public appearances, at all times and in the proper manner as directed by FHC. ATHLETES wishing to be selected for Major Games, agree to sign other agency agreements related to sponsorship and promotion. The athlete player representative will be consulted as part of any major review of the national team training and competition uniform. Decisions on training and competition uniforms will be the responsibility of FHC. All decisions will be made within the guidelines set out by the International Hockey Federation (FIH) and/or Major Games sponsorship and promotions agreements as relevantCKC;
h. j) Participate in reasonable educational or non-commercial, promotional activities as may be requested by FHC CKC or the Government of Canada, provided the ATHLETE Athlete shall be compensated for out-of-pocket expenses and such activities will not exceed the equivalent of two days per year. FHC usually makes such requests for participation and arranges the activities. Unless supplementary compensation is arranged, these activities do not normally involve more than three working days per athlete per year. This is in addition to commercial appearances that the ATHLETE may make on behalf of FHC as described in Section 3(a)(ii) of this Agreement;
i. Supply FHC with biographical information as requested, k) Assign and notify FHC and does hereby assign to CKC the Athlete Assistance Program immediately of any change of address;
j. As a member of FHC and of the National Teams Program, behave in a lawful, courteous and respectful manner and adhere at all times exclusive worldwide right to FHC’s policies and rules relating to Athlete conduct in effect from time to time;
k. Where the ATHLETE receives financial support through the AAP, undertake to fulfill all commitments and responsibilities outlined in the publication Policy and Procedures – Athlete Assistance Program (April 26, 2019), and agree to refund to the Government of Canada any over-contribution paid to the ATHLETE should the ATHLETE’S carding status change, effective the date of such change;
l. Execute any further documents that may be required by FHC to give effect to the undertakings set out in this Agreement;
m. Indemnify and hold harmless FHC from any losses that FHC may incur as a result of the ATHLETE’S failure to satisfy any provision(s) of this Agreement. This indemnification shall survive termination of this Agreement;
n. During or at National Team training camps and competitions, avoid alcoholic consumption to a level that would reasonably be expected to impair use the Athlete’s ability image, name, nickname or other likeness for the promotion of competitive canoeing competitions organized by CKC or in which CKC is a participant. Such assignment shall be in perpetuity. After the athlete has retired from active international competition, use of the image is subject to speakathlete and agent approval, walk, or drive, or cause the Athlete such approval not to behave in a disruptive mannerbe unreasonably withheld;
o. Avoid l) Warrant that he/she has not and will not enter into any promotional or commercial sponsorship or endorsement within the tobacco industry involving the use of banned substances that contravene the rules of the International Olympic Committee (IOC)Athlete’s image, the rules of the International Federation (FIH) and the Canadian Policy on Doping in Sportname, nickname or other likeness;
p. Submit, without prior warning, to unannounced doping-control tests in addition to other prior-notice tests and submit at other times to doping-control testing when requested by FHC, Sport Canada, the Canadian Centre for Ethics in Sport (CCESm) or other authorities designated to do so;
q. Avoid the possession of anabolic drugs and neither supply such drugs to others directly or indirectly nor encourage or condone their use by knowingly aiding in any effort to avoid detection of the use of banned substances or banned performance-enhancing practices;
r. v. Actively participate in all FHC evaluation activities. ATHLETES will co-operate n) Avoid participating in any evaluation competitions where federal government sport policy has determined that such participation is not permitted;
o) Supply CKC with biographical information as requested, and notify CKC in writing immediately of FHCany change of address;
p) As a registered competitor of CKC and as a member of the National Team, behave in particular a courteous and respectful manner and adhere at all times to the High Performance Programs that Standard of Conduct Policy;
q) Affix corporate identification for all National Team Sponsors on his or her paddle and boat, when under the jurisdiction of CKC, as defined in the WRD Individual Athlete Sponsorship Policy and as may be conducted further directed by the Board, Director General and National Team working groups or anyone authorized to act on FHC’s behalf and provide such data as the person conducting the evaluation considers necessary for the proper conduct of the evaluationHead Coach;
w. Avoid any action or conduct that would reasonably be expected r) Comply with the by-laws, rules, regulations and policies of CKC, as adopted and amended from time to significantly disrupt or interfere with a competition or the preparation of any Athlete for a competition.time;
