Common use of Optical Treatment Clause in Contracts

Optical Treatment. Specialty Emergency Services will pay for reasonable and customary expenses incurred for optical treatment. Optical treatment must be provided by an optician, optometrist, or ophthalmologist. Specialty Emergency Services will pay for the following treatment: 3.22.1 One eye test which includes optical consult, sight and vision testing per membership year. 3.22.2 Costs for glasses lenses or contact lenses to correct sight and vision problems such as short or long sightedness per membership year. 3.22.3 Costs for glasses frames, which must be accompanied by the glasses frames prescription or invoice to support the frames request, every second year of membership. All optical treatment, regardless of the extent of treatment, is subject to the benefit and geographical limits stated in the Table of Benefits.
