Common use of Orthodontic Treatment Clause in Contracts

Orthodontic Treatment. The Employer agrees to provide for services relating to orthodontics only to dependent unmarried children of the employee between the ages of six (6) and eighteen (18), on the basis of fifty percent/fifty percent (50%/50%) co-insurance, in accordance with the current Ontario Dental Association Schedule of Fees, up to a lifetime maximum benefit of two thousand, five-hundred dollars ($2,500.00) for each such dependent unmarried child.
