Other Reporting Requirements. The following describes certain other reports required under this Contract:
Other Reporting Requirements. The Contractor shall report to the City, on a monthly basis, all information reasonably requested by the City that is necessary for the City to comply with any reporting requirements imposed by Law, including any requirement that the City maintain a publicly accessible database. In addition, the Contractor agrees to comply with all reporting requirements imposed by Xxx, or as otherwise requested by the City.
Other Reporting Requirements. The MCO must provide the STATE and CMS with the following information in a format and time frame determined by the STATE and CMS. The MCO shall submit information to the effect that no change has occurred since the prior year for reports which require an annual update and where no change has occurred since the prior year. With any new report required under this section, the STATE will provide the MCO the technical specifications for the report at least sixty (60) days prior to the effective date of when the report is to be submitted, unless the STATE determines that a shorter time period is necessary. This provision does not apply to ad hoc reports requested by the STATE. Enrollment and Marketing Materials. Enrollment and Marketing Materials and plans as outlined in this Contract. Service Delivery Plan. Any substantive changes in the Service Delivery Plan previously submitted with the MCO’s Request for Proposal (RFP) response to the most recent MHCP procurement shall be provided by the MCO to the STATE thirty (30) days prior to any changes made by the MCO. The STATE must approve all changes to the MCO’s Service Delivery Plan. Care Coordination and Case Management Systems: By September 15th of the Contract Year, the MCO must provide an updated description of the Case Management System for MSC+ and Care Coordination system for MSHO using a Delegate Review Reporting template developed jointly by the STATE and MCOs. This description shall include, but will not be limited to: A document describing how MSHO care coordination and MSC+ case management is being provided for community, EW and nursing home members by county and population group including whether it is provided through contracts with local agencies or tribes, clinic or provider care systems, community agencies, health plan staff or other arrangements or through a combination of such arrangements; The most recent SNP model of care as submitted to CMS, unless already submitted to the STATE and there has been no change since the submission; Lists and descriptions of entities providing Care Coordination and Case Management contractors, duties of such entities or subcontractors, contracting and delegation arrangements; A description of Care Coordination and/or Case Management screening and assessment tools, timelines and follow up processes; A description of use of protocols for management of chronic conditions including procedures for communication with clinics and physicians; A description of use of Nurs...
Other Reporting Requirements. The Borrower will, and will cause the Parent to, deliver to the Administrative Agent (which shall promptly make such information available to the Lenders in accordance with its customary practice):
(i) within five (5) Business Days after a Responsible Officer of the Borrower or the Parent becomes aware of the occurrence of any Default or Event of Default, a certificate of a Financial Officer of the Borrower or the Parent, as applicable, setting forth the details thereof and the action which the Borrower or the Parent, as applicable, is taking or proposes to take with respect thereto;
(ii) promptly upon the filing thereof with the SEC, copies of all reports with respect to material litigation which the Borrower, the Parent or any Subsidiary of the Parent files with the SEC;
(iii) within five (5) Business Days after the furnishing thereof, copies of all financial statements and reports sent to the stockholders of the Parent generally, and promptly upon the filing thereof with the SEC, notice by electronic mail of the filing of any financial statements and reports which the Parent filed with the SEC (other than reports referred to in Sections 5.01(a)(i), 5.01(a)(ii) and 5.01(c)(ii));
(iv) promptly upon receipt thereof, notice of any downgrade in the S&P Rating or the Xxxxx’x Rating of the Borrower;
(v) simultaneously with the delivery of the certificate referred to in Section 5.01(a)(iii), if any Subsidiary has become or ceased to be a Significant Subsidiary, a revised Schedule II disclosing the Significant Subsidiaries as of the date of such certificate;
(vi) prompt notice of any proposed waiver, amendment, supplement or other modification of any term or condition of the Support Agreement;
(vii) promptly upon the request thereof, such other information and documentation required by bank regulatory authorities under applicable “know your customer” and anti-money laundering rules and regulations (including the Act), as from time to time reasonably requested by the Administrative Agent or any Lender;
(viii) if reasonably requested by any Lender (through the Administrative Agent) or the Administrative Agent, the Borrower will furnish to the Administrative Agent evidence of compliance with the Margin Regulations; and
(ix) from time to time such additional information regarding the business, financial condition or results of operations of the Borrower, the Parent or the Significant Subsidiaries of the Parent as the Administrative Agent and the Lenders may reasonab...
Other Reporting Requirements. This reporting in no way abrogates the Contractor’s responsibility for additional safeguarding and cyber incident reporting requirements pertaining to its unclassified information systems under other clauses that may apply to its contract, or as a result of other U.S. Government legislative and regulatory requirements that may apply (e.g., as cited in paragraph (c) of this clause).

(A) Birth of Child to an Enrollee. The MCO may report to the STATE or the Local Agency the birth of any Child (except those in (2) below) to an Enrollee on a form approved by the STATE, as soon as reasonably possible after the MCO knows of the birth. No later than thirty (30) days after claim adjudication and in a form and manner determined by the STATE, MCO shall on a monthly basis report the birth of a Child or end of pregnancy to an undocumented woman identified by eligibility type “PC” in the capitation payment files.

Grantees must report whether the project prioritizes local hires and whether the project has Community Benefit Agreement, with a description of any such agreement, if applicable. For water and sewer projects, Grantees shall provide the following information to the Department once the project starts, as appliable:
i. National Pollutant Discharge Elimination System (NPDES) Permit Number, for projects aligned with the Clean Water State Revolving Fund
ii. Public Water System (PWS) ID number, for projects aligned with the Drinking Water State Revolving Fund.

Other Reporting Requirements i. A&D (Aging & Disability System): Providers shall have dedicated staff responsible for maintaining the client tracking software, an Internet Service Provider and the appropriate hardware that can support the program. Providers shall be responsible for the following A&D data entry by 4pm PST on the 15th day of the month following the month of service: • Client Intake Sheets for any new clients or any annual or quarterly assessments must be completed in the A&D system within the month of registration date, annual or quarterly update • Updating database with new clients • Adding services and service units to client records
ii. Monthly Performance Report (MPR): The MPR is a tool that is used to report the number of service units, unduplicated clients, and nutrition education units provided to participants. This report is to be completed and submitted to County by the 15th day of the month following the month of service, unless otherwise approved by the County via email to XxXXxxxXxxx@xxxx.xxxxx.xxx.
