Ownership and Related Information Disclosure. 2.1.12.1 The FIDA Plan shall report ownership and related information to NYSDOH, and upon request to the Secretary of Health and Human Services and the Inspector General of Health and Human Services, in accordance with 42 U.S.C. §§ 1320a-3 and 1396b(m)(4) (§§ 1124 and 1903(m)(4) of the SSA).
