Participating Agencies Commitments. 1. Identify a single point of contact or key points of contact within each organization who can provide service information and/or process referrals. 2. Meet on a regular basis to provide program information, service updates and review communication, resident feedback and referral protocols. 3. Hold team meetings as needed to ensure effective care coordination for residents. 4. Share information to the extent that is allowed under Oregon state law, HIPAA regulations as referenced in Section 4 (A) (4) above and their own agency requirements. 5. Provide information to HWS staff regarding resident services and service outcomes. 6. Be available to the Resident Advisory Council to conduct outreach and solicit feedback regarding services delivery.
