Personal Information Form. Please fill out the personal information form attached. It will provide us with some initial information about you and your reasons for seeking our services. We also use this information to evaluate our present efforts and to address current needs. The information you provide is voluntary. If you do not wish to complete any parts of this form, you need not do so. However, we encourage you to use this opportunity to aid our staff in providing you with the best possible assistance.
Personal Information Form. Notwithstanding any other provision of this Section 9, in the event that Holder is the holder of 10% or more of the Company’s outstanding Common Shares, no conversion of any principal or interest of this Convertible Note shall occur until the individuals that would control the votes of any Common Shares held by the Holder (upon conversion of this Convertible Note) shall have registered on the System for Electronic Disclosure by Insiders and provided to the TSX a “Personal Information Form” and such form shall have been cleared by the TSX. The Company shall assist the Holder, with the filing of such form as necessary.
Personal Information Form. Subject to the Company’s compliance with its covenant set forth in Section 3.01(kk), the Lender hereby covenants and agrees to provide a duly completed Personal 23 Information Form if required by the Cboe Exchange within 10 Business Days of becoming an Insider (as defined in the policies of the Cboe Exchange) of the Company.

In connection with the employment of the Employee as a senior officer of the Company, the Employee will complete, and the Company will submit to the applicable regulatory authorities, a “Personal Information Form” in the form prescribed by the Toronto Stock Exchange. The employment of the Employee hereunder is conditional upon the completion of such form and neither the Toronto Stock Exchange nor any applicable securities commission raising any objection to the appointment of the Employee as a senior officer of the Company.

The Employee acknowledges and agrees that the Corporation may be required, from time to time, to submit the PIF or other form of personal information document to securities regulators, stock exchanges or other regulatory bodies. The Employees covenants and agrees to provide an updated PIF or other form of personal information form as may be requested by the Corporation from time to time and consents to the submission of said PIF or other form of personal information form to such securities regulator, stock exchange or other regulatory body as the Corporation may determine. The Employee represents and warrants to the Corporation that all information provided by the Employee in the PIF is accurate as of the date hereof.
