Physician Services. Physician services are health care services a licensed medical physician (M.D. – Medical Doctor or D.O. – Doctor of Osteopathic Medicine) provides or coordinates.
Physician Services. Visits to or by a Physician for a non-Surgical health Services as a Covered Person may require in the treatment of an Injury or Illness.
Physician Services. Contractor agrees to provide the services of Physicians having general and specialized medical education, experience, and expertise meeting the criteria of the Board. Only those Physicians who have been approved by the Board to perform the Services, and who do not have a conflict in providing such Services, shall be assigned work under this Agreement. A conflict exists when the Physician has treated or previously examined the Member or any person in the Member’s immediate family, or if another member of the preferred provider organization or managed care provider to which the Physician belongs has treated or examined the Member or any person in the Member’s immediate family. The Physician services to be provided include (i) the IME of Members who apply for disability retirement benefits, (ii) production of comprehensive narrative medical reports (“Reports”), and may include (iii) testimony at administrative hearings as detailed below.
Physician Services. All Medical Services, which are provided by a licensed physician, including supervision of interns and residents.
Physician Services. Any and all Program Services will be provided independently from any other services provided by your Physician and covered by your private or Medicare health insurance plan (“Plan”) and you will not be billed directly for those services except for any applicable co-payments, deductibles or office visit fees. Program Services are outside Plan coverage or reimbursement. Your Physician provides the following Program Services:
Physician Services a. Physician Recordkeeping; general
i. All physicians shall keep such legible individual records as are necessary to fully disclose the kind and extent of services provided, as well as the medical necessity for those services.
ii. The minimum recordkeeping requirements for services performed in the office, home, residential health care facility, nursing facility (NF), and the hospital setting shall include a progress note in the clinical record for each visit, which supports the procedure code(s) claimed.
iii. The progress note shall be placed in the clinical record and retained in the appropriate setting for the service performed.
iv. Records of Residential Health Care Facility patients shall be maintained in the physician’s office.
v. The required medical records including progress notes, shall be made available, upon their request, to the New Jersey Medicaid/NJ FamilyCare program or its agents.
b. Minimum documentation; initial visit; new patient
i. The following minimum documentation shall be entered on the medical record, regardless of the setting where the examination is performed, for the service claimed by use of the procedure codes for Initial visit – New patient:
1) Chief complaint(s);
2) Complete history of the present illness and related systemic review, including recordings of pertinent negative findings;
3) Pertinent past medical history;
4) Pertinent family and social history;
5) A record of a full physical examination pertaining to, but not limited to, the history of the present illness and including recordings of pertinent negative findings;
6) Diagnosis(es) and the treatment plan, including ancillary services and medications ordered;
7) Laboratory, X-Rays, electrocardiograms (ECGs), and any other diagnostic tests ordered, with the results; and
8) The specific services rendered and/or modality used (for example, biopsies, injections, individual and/or group psychotherapy, and family therapy).
c. Minimum documentation; established patient
i. The following minimum documentation shall be entered in the progress notes of the medical record for the service designated by the procedure codes for ESTABLISHED PATIENT;
1) In an office or Residential Health Care Facility:
a. The purpose of the visit;
b. The pertinent physical, family and social history obtained;
c. A record of pertinent physical findings, including pertinent negative findings based upon (a) and (b) above;
d. Procedures performed, if any, with results
e. Laboratory, X-Ray, electrocard...
Physician Services. During the term of this Management Agreement, neither Group nor any Group Physician shall provide any medical services except in accordance with the terms of this Management Agreement. A Group Physician who terminates employment by Group shall be relieved of this obligation, except that for a period of one (1) year following termination of employment, such Group Physician shall not practice medicine or solicit any Patient of Group served at the Practice Site for the purpose of treating such patient at a medical facility within a three (3) mile radius of such Practice Site. The parties agree that the duration, area and scope of activities restricted hereunder are reasonable and necessary to protect Manager's legitimate business interests. In the event that a court or arbitrator shall determine that this covenant is unenforceable because of its area, duration or prohibited scope of activities, this covenant shall be construed, in a manner consistent with applicable law, to provide the maximum restriction on the activities of Group and Group Physicians. Manager shall not, during the term of this Agreement, contract with any physician or group of physicians to provide services at a Practice Site or within a three (3) mile radius of a Practice Site, unless Manager offers Group the opportunity to Group to provide the services and Group declines to provide Group Physicians or hire new physicians necessary to provide the requested services.
Physician Services. At admission you will select and designate a physician to follow you during your stay. All medications, treatments, and diets are subject to the physician’s direction. If you can’t select an attending physician the facility can refer you to one who has privileges at the facility. Physicians will see you at regular intervals and provide documentation of care as required by law. If you want to speak with your physician, and have questions, concerns, issues we can help you. Physician Services. Physician services are those services rendered by a licensed physician who possesses the appropriate Medicaid specialty requirements when applicable. A psychiatrist must be certified as a psychiatrist by the American Board of Psychiatry and Neurology or the American Osteopathic Board of Neurology and Psychiatry, or have completed a psychiatry residency accredited by the Accreditation Council for Graduate Medical Education (ACGME) or the Royal College of Physicians and Surgeons of Canada.
