Placement and Services. For any student to be placed in an NPS/A, the Superintendent or designee shall develop an individual services agreement based on the student's IEP. Placement and Services. A. Within ten days of the date of this Agreement, the District will convene an Individual Education Plan (IEP) team meeting for the Student. B. During the IEP meeting referenced in Section I.A of this Agreement, the IEP team will review the Student’s psychoeducational evaluation, and consider each of the nine recommendations made by the assessor, any specific placement or accommodation requests or other information submitted by the Complainant and Student. The District will also determine whether the Student needs compensatory and/or remedial services for subjects and/or classes the Student failed during the Spring 2015 semester. If so, the Student’s IEP team will include a plan for providing timely compensatory and/or remedial services in the Student’s IEP, and make such services available by the end of the 2015-2016 school year, including the 2016 Extended School Year. C. Based on the consideration of the information outlined in Section I.B of this Agreement, the District will develop a new IEP based on the individualized needs of the Student that will include the following: 1. The appropriate placement for the Student, including whether it should include participation in a credit recovery program or class; 2. The specific regular and special education and related aids and services, including behavior support services and supplemental support services or tutoring, the Student will receive, including the nature, frequency and duration of each service; 3. The position of the individual(s) responsible for delivering each of the specified services; and 4. A credit recovery plan designed to enable the Student to repeat and/or complete classes the Student failed during the 2014-2015 school year which are needed in order to graduate from high school on time, including, if applicable, Extended School Year services and any applicable recommendations from the evaluation described in Section I.B of this Agreement. The IEP team will develop a reasonable timeline for the credit recovery plan which may extend beyond the 2015-2016 school year. D. The District agrees that any transition goals developed for the Student will be based on the Student’s individual needs and interests, and that transition services, such as career interest assessments and college counseling, will be provided with appropriate accommodations for the Student.

Placement and Services. The District agrees to fund a diagnostic placement for Student at the nonpublic school ("NPS"), The Non-Public School located at (NPS ADDRESS) (the "Non-Public School"), from Student's acceptance into the Non-Public School through the Term. The District further agrees to fund related services for Student provided by the Non-Public School. The related services will be 60 minutes per week of speech and language services, 30 minutes per week of occupational therapy ("OT"), and 30 minutes per month of counseling ("related services"). District agrees to fund Student's placement and related services pursuant to the terms set forth in the Master Contract between the Non-Public School and the District. The District will cease funding tuition and related services for the Non-Public School at the end of the Term, unless the Individualized Education Program ("IEP") team places Student there through the IEP process called for by paragraph f. of this Agreement. Parent agrees to request that the Non-Public School invoice the District directly for the tuition and related services and understands the District will process payment to the Non-Public School directly following receipt and processing of an invoice from the Non-Public School. Parents hereby consent to the District and the Non-Public School exchanging information about Student for purposes of implementing this Agreement and providing FAPE to Student pursuant to its terms, and agree to sign any releases of information or other forms necessary to allow for such exchange of information and placement pursuant to this Agreement. Parents have selected the Non-Public School as a diagnostic placement and understand the District makes no warranty as to the appropriateness, adequacy, safety, and/or fitness of the Non-Public School placement or related services under this Agreement, and that this Agreement is in lieu of the District's obligation to provide Student with a FAPE during the Term. The Parties agree that, other than as stated herein, the District has no obligation to assess Student, develop or review any IEP for Student, or to provide Student with any special education and/or related services through the Term, other than as stated in this Agreement. If Non-Public School becomes unavailable to Student for reasons outside the Parties' control, prior to the end of the Term, Parents will provide written notice to the District's Special Education department within 3 school days of said removal. To provide highly qualified instructors for the courses being taught; ii. To identify students who are not college ready as stated in HB 5; iii. To provide professional development and resources required to teach the mathematics and English language arts courses; iv. To identify successful completion of the course(s) on the student transcripts as determined by the State of Texas PEIMS number; v. To provide curriculum for the course that is consistent with HC Student Learning Outcomes; vi. To provide assistance with admission, enrollment, and financial aid applications; E. ISD agrees to the following for the college preparatory mathematics course: i. To teach a math course designed to focus on college mathematics (algebraic or non‐algebraic) concepts; ii. Require students to meet college readiness scores on the TSI Assessment; iii. To meet regularly with HC faculty. F. ISD agrees to the following for the college preparatory English language arts course: i. To teach an integrated Reading and Writing course that focuses on critical reading and college‐level writing; ii. Require students to meet college readiness scores on the TSI Assessment; iii. To meet regularly with HC faculty.

  • Programs and Services Every aspect of the service you provide is considered part of your program, and therefore it must be accessible to individuals with disabilities. This includes parking lots, service counters and spaces, transportation (shuttles, etc.), agendas, flyers, emails, online services, phone calls, meetings, celebrations, classes, recreational activities and more. The guidance in this document is primarily intended to help you provide accessible programs by providing you with the tools to: ▪ survey facilities and identify common architectural barriers for people with disabilities; ▪ identify common ADA compliance problems in your communications and activities; and ▪ remove barriers and fix common ADA compliance problems in these areas. Your programs can be broken into three main categories, (Communications, Facilities, and Activities) which will be covered in more detail below.

  • Appointment and Services (a) The Trust appoints the Administrator to provide the administrative services set out in Appendix A to this Agreement (the “Administrative Services”) for the benefit of the Trust and the Funds.   • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Autism Services This plan covers the following services for the treatment of autism spectrum disorders. • Applied behavior analysis when provided and/or supervised by an individual licensed by the state in which the service is rendered. See the Summary of Medical Benefits for the amount that you pay. • Physical therapy, occupational therapy, and speech therapy services when rendered as part of the treatment of autism spectrum disorder. A benefit limit will not apply to these services. • Psychological and psychiatric services, and prescription drugs are also covered. See Behavioral Health Services and Prescription Drugs and Diabetic Equipment or Supplies for additional information. Coverage for autism spectrum disorders does not affect any obligation of a school district, a state or other governmental entity to provide services to an individual under an individualized family service plan, an individualized education program, or similar services required under state or federal law. Services related to autism that are furnished by school personnel are not covered under this plan.

  • Additional Products and Services Subject to the allocation of funds, the CPO may add similar equipment, supplies, services, or locations, within the scope of this Agreement, to the list of equipment, supplies, services, or locations to be performed or provided by giving written notification to Contractor. For purposes of this Section, the “Effective Date” means the date specified in the notification from the CPO. As of the Effective Date, each item added is subject to this Agreement, as if it had originally been a part, but the charge for each item starts to accrue only on the Effective Date. In the event the additional equipment, supplies, services, or locations are not identical to the items(s) already under this Agreement, the charges therefor will then be Contractor’s normal and customary charges or rates for the equipment, supplies, services, or locations classified in the Fees and Costs (Exhibit “F”).

  • New Services If, within ninety (90) days after the Distribution Date, a Party desires the other Party to provide additional or different services which such other Party is not expressly obligated to provide under this Agreement (excluding, for the avoidance of doubt, any Additional Services or Service Adjustments, the “New Services”), then such Party will provide a written change request (in the form agreed by the Parties) to the other Party within ninety (90) days after the Distribution Date. The Party receiving such request shall negotiate in good faith to provide such New Service; provided, however, that no Party shall be obligated to provide any New Services, including because the Parties are unable to reach agreement on the terms thereof (including with respect to Service Charges therefor). If the Parties agree to any such New Service, then the Parties shall document such terms in a Service Schedule to be incorporated in Schedule A or Schedule B, as applicable. The Service Schedule shall describe in reasonable detail the nature, scope, service period(s), termination provisions and other terms applicable to such New Services. Each supplement to the applicable Service Schedule, as agreed to in writing by the Parties, shall be deemed part of this Agreement as of the date of such agreement and the New Services set forth therein shall be deemed “Services” provided under this Agreement, in each case subject to the terms and conditions of this Agreement. The Parties shall in good faith determine any costs and expenses, including any start-up costs and expenses, which would be incurred by the Provider in connection with the provision of such New Service, which costs and expenses shall be borne solely by the Recipient.
