As a guest with a pet staying at The Bird House Bungalow, I acknowledge and understand that there are certain risks associated with having a pet residing with me at any hotel or vacation property. These risks include, but are not limited to: injuries or illnesses suffered by my pet while on the property or as a result of my pet being on the property, and any illnesses contracted by any pet not FULLY vaccinated. In consideration of allowing the pet to stay in The Bird House Bungalow, I hereby agree to ASSUME ALL RISKS associated with my pet’s presence on the premises of The Bird House Bungalow rental property. Additionally, I agree to HOLD HARMELESS and RELEASE, DEFEND, AND INDEMNIFY, THE BIRD HOUSE BUNGALOW and all its respective insurance carriers, staff, volunteers, representatives, and its owner, FOR ANY AND ALL LIABILITY and/or claims for illness, injury or death to persons or damage to property arising from my pet’s stay at The Bird House Bungalow. By signing this release, I agree not to sue any released party and agree I am releasing any right to make a claim or file a lawsuit against any released party. I agree that any claims arising from the pet’s presence at The Bird House Bungalow, shall be governed by Florida law and exclusive jurisdiction of any claim shall be in the district court for Sarasota County or in the Federal court for the State of Florida. When I must leave my pet alone, it will be crated (complimentary crate available) or confined to a single room. For the safety of my pet - when I must confine, I will confine on the ground floor only. On a DAILY basis, I will pick up and properly dispose of (in green covered can) any pet waste. (Bags are provided.) When my pet is outside in the yard, it will be either on its leash or accompanied by me at all times I will use the Park directly across 00xx Xxxxxx for walks as frequently as possible. If an accident occurs inside the house, I will clean it as quickly as possible to avoid ‘setting’. If I must have my pet sleep or sit with me, I will use my own personal bedding/blanket/sheet to protect the furniture, and I will wash it frequently. As This Assumption of Risk, Release and Medical Authorization Agreement ("Agreement") is entered into for the benefit of Xxxxxxx Amateur Ski Racing Association ("MASRA"), Mt. Hood Xxxxxxx Oregon Ltd., the other organizers of ski races and the other ski areas where races may be held and, in each case, their respective officers, directors, partners, members, owners, employees, agents, volunteers, landowners, affiliated entities, successors and assigns (collectively referred to in this Agreement as the "Protected Parties") as a guest condition to the Racer participating in ski races, race training, free skiing, snowboarding, special events and all other activities sponsored or conducted !" any of the Protected Parties or in which any of the Protected Parties may participate (collectively referred to in this Agreement as "Activities") during the ski season indicated above. The undersigned acknowledges that skiing and participation in the Activities are HAZARDOUS activities which carry with a pet staying at The Bird House Bungalowthem significant risk of serious personal injury, death or property damage. I acknowledge and understand also know that there are certain natural, mechanical and environmental conditions and risks associated which independently or in combination with having the Activities may cause property damage or severe or even fatal injuries to me or others. I have made a pet residing with voluntary choice to participate in the Activities and, in the case of a parent or guardian, to allow my child or xxxx to participate in the Activities. I agree to accept all responsibility for the risks, conditions and hazards which may occur whether or not they are now known or contemplated by me. Acknowledgement and Assumption of Risks I agree that I alone am responsible for the safety of myself and, in the case of a parent or guardian, of the Racer while participating in the Activities and providing, utilizing and maintaining the equipment necessary for the safe enjoyment of skiing and participation in the Activities by me at any hotel and, in the case of a parent or vacation propertyguardian, by the Racer. These risks include, but I specifically acknowledge that the Protected Parties are not limited to: injuries responsible for my safety or, in the case of a parent or illnesses suffered by my pet while on guardian, the property or as a result safety of my pet being on the propertyRacer. I currently have, and any illnesses contracted by any pet not FULLY vaccinatedI agree to maintain throughout the entire ski season specified above, valid and sufficient medical and accident insurance covering me and in the case of a parent or guardian, my child or xxxx. I understand that this is my sole responsibility and I release all Protected Parties from providing this coverage. As a guest with a pet staying at The Bird House BungalowI understand that, I acknowledge and understand that there are certain risks associated with having a pet residing with me at -If my child's behavior is disruptive or harmful in any hotel way, tournament officials or vacation propertyreferees reserve the right to expel the participant from the tournament without refund. These risks include, but are not limited to: injuries or illnesses suffered by my pet while on the property or as a result of my pet being on the property, and any illnesses contracted by any pet not FULLY vaccinated. In consideration of allowing the pet to stay in The Bird House Bungalow, I hereby agree to ASSUME ALL RISKS associated with my pet’s presence on the premises of The Bird House Bungalow rental property. Additionally, I agree to HOLD HARMELESS and RELEASE, DEFEND, AND INDEMNIFY, THE BIRD HOUSE BUNGALOW and all its respective insurance carriers, staff, volunteers, representatives, and its owner, FOR ANY AND ALL LIABILITY and/or claims for illness, injury or death to persons or damage to property arising from my pet’s stay at The Bird House Bungalow. By -By signing this releaseform, I agree not to sue hold Xxxxx Youth Lacrosse Club, coaching staff members, officials or any released of their members or trustees, liable for any injuries that may occur to my child while participating in the team’s activities or while on Xxxxx Schools property. -I authorize the coaching staff or tournament officials /staff to select and secure medical attention including any medical transport as may be necessary for my child as a result of injuries or other events requiring emergency care while I/we are not in attendance at such event. - I understand there is no re-scheduling of games if they are cancelled due to weather. - I realize that Xxxxx Youth Lacrosse and its organization do NOT provide insurance protection and that my child is playing at their own risk.
