POLITICAL CONTRIBUTION DISCLOSURE AFFIRMATION. I FURTHER AFFIRM THAT: The beneficiary affirms that it is aware of, and will comply with, the provisions of Sections 14-101 through 14-108 of the Election Law Article of the Annotated Code of Maryland, that a person making or having a single contract with a single governmental entity involving cumulative consideration of at least $200,000 shall file an initial statement, and semi-annual statements as applicable, with the State Board of Election disclosing applicable contributions of $500 or more, or the lack thereof.
