Common use of Pre-authorization for Emergency Care Clause in Contracts

Pre-authorization for Emergency Care. If the Insured Person has been admitted into Hospital for Emergency Care, We or Our TPA shall be contacted to request pre-authorization for availing the Cashless Facility for that Emergency Care within 24 hours of commencement of Hospitalisation. Each such request must be accompanied by all the following details: (i) The Health Card We have issued to the Insured Person; (ii) The Policy Number; (iii) Name of the Policyholder; (iv) Name and address of Insured Person in respect of whom the request is being made; (v) Nature of the Illness/Injury and the treatment/surgery required; (vi) Name and address of the attending Medical Practitioner; (vii) Hospital where treatment/surgery is being taken;
