An authorization does not necessarily guarantee the payment for any services rendered to the consumer.

Prior Authorization Requirements. In some cases, MVP may require that your prescribing provider satisfy MVP Prior Authorization Requirements before a prescription is filled at the pharmacy. Drugs that must be Prior Authorized before they are filled are identified on the Formulary. They are also listed by therapeutic categories on our Formulary. MVP notifies Providers, in writing, when we change these requirements. New FDA approved prescription medications are subject to Prior Authorization for a minimum of six (6) months. All compounded prescriptions over $250 require Prior Authorization. Compounds containing non-FDA approved drugs may also require Prior Authorization.

If the Family Leave is foreseeable, the employee must provide the agency/department with thirty (30) calendar days notice of his or her intent to take Family Leave. 2.
