Reassessment. IDT staff shall routinely reassess, and as appropriate update, all of the sections in the member’s comprehensive assessment and MCP as the member’s long-term care outcomes change. At a minimum, the reassessment and MCP review shall take place no later than the end of the sixth month after the month in which the previous comprehensive assessment was completed. The reassessment shall include a review of previously identified or any new member long-term care outcomes and supports available. At a minimum: a. The most appropriate IDT staff shall conduct this reassessment; and b. The IDT staff conducting the re-assessment shall ensure that the other IDT members are updated and involved as necessary on the reassessment; c. When a complex medication regime or behavior modifying medication or both are prescribed for a member, the requirements in C.1.c.ii. shall be met; d. The entire IDT shall participate in the annual reassessment that is done no later than the end of the twelfth month after the previous comprehensive assessment was completed. In addition, the most appropriate IDT staff shall conduct a reassessment whenever there is: e. A significant change in the member’s long term care or health care condition or situation; or f. A request for reassessment by the member, the member’s legal decision maker, the member’s primary medical provider.
