Registration Statement Questionnaire Completed. The Buyer has completed or caused to be completed the Registration Statement Questionnaire attached here to as part of Appendix I, for use in preparation of the Registration Statement, and the answers thereto are true and correct as of the date hereof and will be true and correct as of the effective date of the Registration Statement, or as applicable, any amendment thereto, as will notify the Company immediately of any material change in any such information provided in the Registration Statement Questionnaire until such time as Buyer has sold all of its Conversion Shares and Warrant Shares or until the Company is no longer required to keep the Registration Statement effective.
