Report Unauthorized Use. Business Associate agrees to notify Covered Entity of any use or disclosure of PHI by Business Associate, or its third party subcontractors or agents, in violation of this Agreement of which Business Associate becomes aware. Business As- sociate shall make such notification in writing, to the individuals designated as contacts by Covered Entity in paragraph 5.6 below, within ten (10) business days of having been made aware of the unauthorized use or disclosure.
