Common use of Reporting Fraud, Xxxxx and Abuse Clause in Contracts

Reporting Fraud, Xxxxx and Abuse. The Contractor will report, in writing, all cases of suspected fraud, waste and/or abuse involving the programs administered by AHCCCS to the AHCCCS Office of the Inspector General (“OIG”). To report fraud, waste and/or abuse, the Contractor will complete the AHCCCS “Referral for Preliminary Investigation” form and submit it to AHCCCS-OIG with a copy sent to AHCCCS-DFSM.
