Reporting Timeframes. All quality measures will be reported based on services provided during the Demonstration Year. If the State fails to report by the requested deadline or does not provide a reasonable explanation for delayed reporting, the State may be subject to corrective action for failing to report quality measures. Inaccurate or incomplete reporting, or failure to make timely corrections following notice to resubmit data may lead to termination from the Demonstration. The State must provide an attestation to the completeness and accuracy of the data reported. The data reported will be validated and is subject to audit.
Reporting Timeframes. By February 1st each year, signatory agencies will make available their fire occurrence and perimeter data (e.g., GIS layer, Excel spreadsheet, database table) for fires within their DPA for all jurisdictions for the preceding year. The data will be forwarded to each agency’s respective region/state headquarters. Each agency can request at any time from a DPA agency an export of all fire occurrences to date within their fire occurrence database to meet agency policies and direction. Failure to meet this for 3 instances in a rolling 12 month period, will result in the Supplier being put on a 6 month corrective plan to resolve and monitor performance. Reporting Timeframes provider and lab differences: In some awardee jurisdictions, reporting timeframes for select diseases differ depending on whether reported by providers or labs. Awardees are requested to ensure that calculations of timeliness of reporting for each case of disease are compared against the appropriate required timeframe. Core Public Pre-Incident Response Note: for cases in which both a provider and a lab report the same case of disease, awardees should count the first instance of reporting the case for the purpose of this performance measure.

The MCO is responsible to inform the Department within 5 business days of the due date if an extension is necessary beyond the required dates. Failure to submit any document, including the attestation form, that the Department finds necessary to calculate and verify the requested Medicaid reimbursement will forfeit the MCO’s right to reimbursement. Failure to submit all of the requested documents by the due dates may result in the reimbursement being delayed.
