Common use of Request for Exclusion Clause in Contracts

Request for Exclusion. “Request for Exclusion” refers to a timely, written, opt- 23 out request signed by a Class Member and submitted to the Settlement Administrator with the following 24 information: (1) the Class Member’s full name and current mailing address, (2) the last four digits of the 25 Class Member’s social security number, and (3) an express statement that the Class Member wishes to 26 be excluded from the terms of the Agreement.

Request for Exclusion. “Request for Exclusion” refers to a timely, written, opt- 23 opt-out 27 request signed by a Class Member and submitted to the Settlement Administrator Administrator, within sixty (60) 28 calendar days of Notice Mailing, with the following 24 information: (1) the Class Member’s full name and 1 current mailing address, (2) the last four digits of the 25 Class Member’s social security number, and (3) an 2 express statement that the Class Member wishes to 26 be excluded from the terms of the Agreement.

Request for Exclusion. “Request for Exclusion” refers to a timely, written, opt- 23 19 out request signed by a Class Member and submitted to the Settlement Administrator with the following 24 20 information: (1) the Class Member’s full name and current mailing address, (2) the last four digits of the 25 21 Class Member’s social security number, and (3) an express statement that the Class Member wishes to 26 22 be excluded from the terms of the class settlement contained within this Agreement.
