Respondent must ensure. Form A, Breast and Cervical Cancer Services (BCCS) Program Certification is completed in its entirety at the time of applications and must annually certify that they do not perform or promote elective abortions and do not affiliate with any entity that does, as directed by HHSC, and for each program with TMHP and HHSC.
