Return or Destruction of PHI. Upon termination of this HIPAA BAA, Jotform will return to Customer all PHI received from Customer or created or received by Jotform on behalf of Customer which Jotform maintains in any form or format, and Jotform will not maintain or keep in any form or format any portion of such PHI. Alternatively, Jotform may destroy all such PHI and provide written documentation of such destruction.
