SAMPLE DOCUMENTATION AND SHIPMENT PROCEDURES. Chain-of-custody records are used to document sample collection, analyses required, sample custody, and transportation to the analytical laboratory for analysis. All samples will be accompanied by a chain-of-custody record. A separate form will be completed, signed, and transported with each cooler containing samples to the laboratory. The chain- of-custody record identifies the contents of each sample cooler, the analyses to be performed, and maintains the custodial integrity of the samples. Corrections on sample forms can be made by placing a single line through the mistake and initialing and dating the change. The correct information would then be entered above, below, or after the mistake. Generally, a sample is considered to be in someone's custody if it is either in someone's physical possession, in someone's view, or locked and kept in a secured area that is restricted to authorized personnel. Until the samples are transported to the individual laboratory, the custody of the samples is the responsibility of the sampling team. A copy of the original chain-of-custody records will be included in the project report. All samples collected will be labeled in a clear and precise way for proper identification in the field and for tracking in the laboratory. The samples will have preassigned, identifiable, and unique numbers. At a minimum, the sample labels will contain the following information: site name, sample location and depth, date of collection, analytical parameter(s), any method of sample preservation, and sampler’s name.
