Service providers' Confidentiality Declarations. every service provider (provider employee, subcontractor or subcontractor's employees) shall be obligated to sign, prior to commencing work under the present Procedure, on a confidentiality declaration. The declaration shall be accompanied with the signature of the provider (in addition to that of the service provider). The confidentiality declarations shall be submitted to the Authority Representative; the declaration template is enclosed herewith, hereinafter referred to as Form 2.
