SERVICE REGION. The Contractor’s Service Region is as follows: Region 5 - Central.
SERVICE REGION. The Contractor’s Service Region is as follows: Region 2 - Iron Range.
SERVICE REGION. The Contractor’s Service Region is as follows: Region 6 - Twin Cities Metro.
SERVICE REGION. The Contractor shall:
(1) Provide NEMT services in Service Region 3B which consists of Snohomish County. The Contractor shall provide Transportation and related services to all eligible clients living within the designated service region; to the extent these services are necessary to support access to authorized healthcare services.
(2) Provide NEMT services to eligible clients of other service regions in response to requests from Contractors responsible for those service regions. Such requests shall be consistent with the intent of 3.b. below.
(3) Ensure all trips provided outside of a client’s local community meet the requirements as set forth in the NEMT portion of WAC Chapter 388-546, when required healthcare services are not available within the service region, or border cities designated by DSHS.
(4) Coordinate out-of-state transportation requests with the DSHS Transportation Program Manager, or designee. Upon DSHS’s authorization, the Contractor shall arrange destination and return trips out-of-state when DSHS has determined that required services are not available within the state or designated border cities.
SERVICE REGION. The Contractor’s Service Region is as follows: Region 7 - Southeast.
SERVICE REGION. Republic of Korea
SERVICE REGION. The Contractor shall:
A. Provide NEMT services in Service Region which consists of County. The Contractor shall provide Transportation and related Services to all eligible clients living within the designated service region; to the extent these services are necessary to support access to authorized healthcare services.
B. Provide NEMT services to eligible clients of other service regions in response to requests from Contractors responsible for those service regions. Such requests shall be consistent with the intent of section 3, County of Residence, of this exhibit.
C. Ensure all trips provided outside of a client's Local Community meet the requirements as set forth in the NEMT portion of WAC Chapter 182-546 when required healthcare services are not available within the service region, or border cities designated by HCA.
D. Coordinate out-of-state transportation requests with the Transportation Program Manager or designee. Upon HCA’s authorization, the Contractor shall arrange destination and return trips out-of-state when HCA has determined that required services are not available within the state or designated border cities.
