Provider certifies that all services billed or reported by Provider are within the scope of Provider's license and are performed personally by Provider and permitted by BCBSM policy, except as otherwise authorized and communicated by BCBSM, and are submitted in accordance with the terms and conditions of the Members' Certificates.

Provider will adhere to all BCBSM published guidelines for the provision and billing of Covered Services to Members.
