The Sick Leave Bank Guidelines clause establishes the rules and procedures for the creation, management, and use of a collective pool of sick leave days contributed by employees. SICK LEAVE BANK GUIDELINES. Eligibility and admission to the Sick Leave Bank is limited to teachers who are tenured in the Lincoln School Department and who have agreed, at the time of initial eligibility, to contribute one (1) cumulative sick day annually, per Article IX, Section 1, of the Collective Bargaining Agreement.
SICK LEAVE BANK GUIDELINES. A. In the event of a catastrophic illness or medical event, all certified employees shall have access to the S.L.
B. Employees may contribute days voluntarily but these days shall not be refundable. If on October 1st of each year the S.L.B has fallen below 30 days, each employee with 15 or more sick days shall contribute one day to the S.L.B.
SICK LEAVE BANK GUIDELINES. The guidelines set forth the criteria and conditions relative to the Districts Sick Leave Bank.
SICK LEAVE BANK GUIDELINES.
A. A Sick Leave Bank (“S.L.B.”) shall be maintained for hospitalization, illness or injury of a teacher for conditions such as, but not limited to cancer, heart attack, stroke, or other catastrophic illnesses or medical events. The S.L.B. is not intended for parental leave that does not involve a catastrophic illness or injury to the teacher. In the event a teacher suffers a catastrophic illness or medical event, the teacher shall have access to the S.L.B.
B. Each teacher shall contribute ½ day of sick leave by October 15th of each contract year, until the maximum amount of 200 contributed sick leave days has been reached. Once the 200 sick leave day maximum has been reached, donations will stop until the contributed sick leave days fall below the maximum. Contributed days into the S.L.B. shall not be refundable.
C. A Usage Control Board of Review consisting of (2) two certified employees appointed by the Association and (1) one administration employee appointed by the Superintendent shall be established to administer the S.L.B.
D. Original requests are limited to no more than 30 days in one school year per teacher. Subsequent requests must be approved by a majority of the usage control board. A grant will be considered only after the teacher has used all of his/her accumulated sick leave days, all possible advances of sick leave days, all available personal leave, and is not eligible for disability leave under the Ohio State Teacher Retirement system.
E. Determinations of applying these and any other policies and guidelines shall be made by the (3) three member sick leave Usage Board whose decision shall be final and not subject to the grievance procedure contained in this agreement.
F. A S.L.B. application form is attached hereto as Appendix C.
SICK LEAVE BANK GUIDELINES. 1. The sick leave bank is open to all members of the bargaining unit represented by the Susquehanna Community Education Association. Professional employees may elect to become part of the sick leave bank. Professional employees who do not elect to become part of the sick leave bank are not eligible for its benefits.
2. The District shall receive written authorization from each professional employee desiring to become a member of the sick leave bank to deduct sick leave from his own accumulated sick leave.
3. Deductions of sick leave shall be limited to one (1) day per school year. Such assessment shall be made prior to the first in-service day whenever possible. Each time an assessment is made of the professional employees, the District shall also contribute forty (40) days. Assessments will continue until the number of days in the sick leave bank exceeds five hundred (250), and reassessment will occur when the number of days in the sick leave bank is less than three hundred (150).
4. New professional employees to the District who wish to opt into the sick leave bank shall be assessed one (1) day in the first year of employment in the District, and then shall continue to be assessed as everyone else.
5. Employees who initially opt not to join the sick leave bank may reconsider their decision at a later date provided that they contribute the equivalent number of days to the sick leave bank that they would have contributed had they been a member of the sick leave bank when they were first eligible.
6. Written verification of illness shall be a prerequisite for the committee to consider the granting of additional sick leave from the sick leave bank. Such verification may need to be made by the District physician, should the Superintendent make that determination.
7. Request for consideration from the sick leave bank committee shall be made on the requisite form.
8. The committee shall consist of the Superintendent and two (2) Association members named by the president of the Association.
9. Prior to being eligible for days from the sick leave bank, all professional employees must exhaust their own accumulated sick leave and personal leave.
10. In special circumstances, the committee may, at its discretion, award days from the sick leave bank not to exceed fifty (50) days per person during any school year. SICK LEAVE BANK GUIDELINES. The basic purpose of this Bank is to alleviate the effects of catastrophic illness upon MVCCSSA members. In accordance with this purpose, the Sick Leave Bank shall not be used for single day occurrences.
