SPECIALTY CARE PROVIDER. A duly licensed physician who has been successfully credentialed by ACDE and who has entered into an agreement to provide Specialty Care Services to Members in accordance with the referral and preauthorization requirements of the Provider Manual.
SPECIALTY CARE PROVIDER. A duly licensed physician who has been successfully credentialed by HMO and who has entered into an agreement with HMO to provide specialty care services to Members referred to such Specialty Care Provider.
