Subcontractor T&M Change Order Request. Subcontractor shall submit upon completion of the changed work (or more frequently if agreed by Contractor) a complete T&M change order recap which shall detail the labor, materials and equipment charges as well as all agreed markups. Attached to such recap shall be copies of all Daily Worksheets signed by Contractor’s Superintendent as well as supporting documentation of materials and equipment costs. Contractor shall have the right to request additional documentation, if necessary to verify quantities and costs.
