Subscriber Identification Sample Clauses

Subscriber Identification. The Provider requires you to register and provide certain data. In consideration of the use of the Service, you represent and warrant that: (a) the information about yourself is true, accurate, current, and complete (apart from optional items) as required by the registration forms (“Registration Data”) and (b) you will maintain and promptly update the Registration Data to keep it true, accurate, current and complete. Each time you update such information, you represent and warrant that such information is true, accurate, current, and complete. You agree to be bound by any affirmation, assent or agreement you transmit through the Internet, including but not limited to any consent you give to receive communications from the Provider solely through electronic transmission. You agree that, when you click on an “I agree,” “I consent” or other similarly worded “button” or entry field with your mouse, keystroke or other computer device, your agreement or consent will be legally binding and enforceable and the legal equivalent of your handwritten signature. If you provide any information that is untrue, inaccurate, not current or incomplete, or we suspect, in our sole discretion, that such information is untrue, inaccurate, not current or incomplete, we have the right, in our sole discretion, to suspend or terminate your account and refuse all current and/or future use of the Service.

Subscriber Identification. Ancillary Provider shall request Subscriber to present Subscriber’s BCBSTX identification card each time Subscriber seeks Covered Services. For patients claiming enrollment in BCBSTX, Ancillary Provider shall request eligibility information by telephoning BCBSTX or by other electronic means established by BCBSTX. However, obtaining information concerning eligibility at the time of service by Ancillary Provider as described in this Agreement is not a verification and does not guarantee payment by BCBSTX in the event that Subscriber is later determined to have been ineligible for benefits at the time of service.
Subscriber Identification. Customer agrees, upon Xxxxxxxx’s request, to promptly supply Provider with the identity of a Subscriber that was the source of any reportedly illegal call traffic Customer routed to Provider. Customer also agrees, if requested, to remove the Subscriber’s traffic from Provider’s route within twenty-four (24) hours of request. If traffic is not removed as requested, Customer’s route will be closed without notice and without liability on the part of Provider until such traffic has been removed.
