Supported Transitional Living Services. Contractor shall be entitled to invoice DSHS monthly in an amount corresponding to the actual hours of STL Services provided during the applicable month, at rates and subject to the limitations set forth on Exhibit B. This amount shall cover the Services of Contractor Staff in providing Supported Transitional Living Services. This payment includes Chaperone Services, which shall be provided by Contractor Staff.
Supported Transitional Living Services. Contractor shall communicate and collaborate with SCC program and fiscal managers, assigned SOTPs, Corrections Specialists, other stakeholders and the Residents to support the development and implementation of each Resident’s Treatment Plan, PBSP and goals and in reviewing Resident progress. Contractor will collect data at the request of the SOTP and RCTT to assist in the development and implementation of the PBSP. Contractor shall provide STL Services in accordance with the Resident’s PBSP as instructed by the RCTT and will report on the Resident’s progress under the PBSP at the monthly RCTT meetings. Contractor Personnel providing STL Services shall be scheduled in a manner that complies with minimum Resident: Staff Ratios. STL Services shall include, but are not limited to, the following:

Contractor shall be entitled to invoice DSHS monthly in an amount corresponding to the actual hours of STL Services provided within Contractor’s LRA during the applicable month, at the rates and subject to the limitations set forth on Exhibit B, Rate Schedule. This amount shall cover the Services of Contractor Staff, including Contractor’s Supervisor, in providing Supported Transitional Living Services, including Chaperone Services. Contractor shall not charge SCC for STL Services provided by Staff when they are out of the Residence providing Chaperone Services, since these are covered in the Chaperone Services charge, below.
