Termination for Chronic Issues. The County has the right to begin the Dispute Resolution process if the Service Provider chronically fails to meet the established Service Levels. The determination for chronic failure is as follows: (a) has any Service Tower reported as “At Risk for more than three (3) out of six (6) consecutive reporting measurement periods or (b) has any Service Tower reported as “Missed” for more than two (2) out of twelve (12) consecutive reporting measurement periods.
