TERMINATION OF AGREEMENT BY. THE HOSPICE PROGRAM The hospice program may terminate this agreement by giving the Director, DHA, or designee, written notice of such intent to terminate at least six months in advance of the effective date of termination. If the hospice program permanently or temporarily ceases to furnish services to the community, the agreement terminates effective with the date the program goes out of business.
