THE BOARD agrees. A. to provide instructional services, job preparation, and supervision as appropriate for the Vocational Instructional Program (see addendum).
B. to provide transportation to and from the Vocational Instructional Program.
C. to solicit evaluation/feedback on individual student performance and overall program effectiveness.
THE BOARD agrees. A. It shall provide the same opportunities for inservice training for XXXXX PRESCHOOL staff involved in teaching ESE students as are provided to THE BOARD staff members.
B. It shall provide consultation from ESE staff upon request from XXXXX PRESCHOOL staff.
C. It shall provide evaluation and transition planning for ESE students aged 3-5 preparing to articulate from XXXXX PRESCHOOL into the Sarasota County School District educational programs.
THE BOARD agrees. A. It shall provide consultation from its Exceptional Student Education staff upon request from PINNACLE ACADEMY.
B. It shall provide copies of any relevant reports or evaluation data relating to eligible ESE students upon receipt of an authorization for release of records signed by the ESE student’s parent(s). Such data shall be maintained by PINNACLE ACADEMY in a manner consistent with applicable Federal and state regulations relating to confidentiality and access to student records.
THE BOARD agrees. A. It shall provide the same opportunities for inservice training for THE FLORIDA CENTER staff involved in teaching ESE students as are provided to THE BOARD staff members.
B. It shall provide consultation from ESE staff upon request from THE FLORIDA CENTER staff.
C. It shall provide evaluation and transition planning for ESE Pre-K students aged 3- 5 preparing to articulate from THE FLORIDA CENTER Program into the Sarasota County School District educational programs.
THE BOARD agrees. A. It shall provide consultation from its Exceptional Student Education staff upon request from CARLTON PALMS.
B. It shall provide copies of any relevant reports or evaluation data relating to eligible ESE students upon receipt of an authorization for release of records signed by the ESE student’s parent(s). Such data shall be maintained by CARLTON PALMS in a manner consistent with applicable Federal and state regulations relating to confidentiality and access to student records.
C. It shall provide evaluation and transition planning for all ESE students preparing to transfer from the CARLTON PALMS into the Sarasota District Schools.
THE BOARD agrees. A. It shall provide the same opportunities for inservice training for THE LILY SCHOOL staff involved in teaching ESE students as are provided to THE BOARD staff members.
B. It shall provide consultation from ESE staff upon request from THE LILY SCHOOL staff.
C. It shall provide evaluation and transition planning for ESE students aged 3-5 preparing to articulate from THE LILY SCHOOL into the Sarasota County School District educational programs.
THE BOARD agrees. A. It shall provide the same opportunities for inservice training for CHILDREN FIRST staff involved in teaching ESE students as are provided to THE BOARD staff members.
B. It shall provide consultation from ESE staff upon request from CHILDREN
C. It shall provide evaluation and transition planning for ESE students aged 3-5 preparing to articulate from CHILDREN FIRST Program into the Sarasota County School District educational programs.

D. To require all participating students subscribe to the school accident insurance coverage and that evidence of this coverage be presented for each student to The Provider if requested.
THE BOARD agrees. A. It shall provide reasonable assistance and consultation as needed to RESIDENTIAL TREATMENT PROGRAM staff to support the ESE educational program for ESE students including completion of the required ESE Matrix of Services for each student.
B. It shall provide copies of any relevant reports or evaluation data relating to eligible ESE students upon receipt of an authorization for release of records signed by the ESE student’s parent(s). Such data shall be maintained by RESIDENTIAL TREATMENT PROGRAM in a manner consistent with applicable Federal and state regulations relating to confidentiality and access to student records.
C. It will provide opportunities for RESIDENTIAL TREATMENT PROGRAM staff to participate in school district training and inservice activities related to ESE students.
THE BOARD agrees. To provide an appropriate space for CYD pre-approved meetings and/or activities at any school location where CYD services are provided.
