Common use of The Parent unreservedly accepts Clause in Contracts

The Parent unreservedly accepts. full responsibility as a Parent to ensure that their Child/children has/have been properly immunized against whooping cough, diphtheria, tetanus and polio and vaccinated against tuberculosis, measles, mumps, rubella and Hepatitis prior to enrolment proof of which must be furnished by the Parent upon request.
