THE UNDERSIGNED HEREBY. ENROLLS IN and adopts The PLAN of Massachusetts and Rhode Island Third Party Special Needs Pooled Trust Agreement dated April 20, 2005, and amended on September 30, 2016, which is incorporated herein by reference.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releases and each of them from any loss, liability, damage or cost they may incur due to the presence of the undersigned in, upon or about the YMCA premises or in any way observing or using any facilities or equipment of the YMCA or participating in any program affiliated with the YMCA whether caused by the negligence of the releases or otherwise.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur due to an injury or accident.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS SwiftKick, its directors, officers, employees, volunteers and agents, and each of them, from any loss, liability, damages or costs they may incur, whether caused by the negligence, active or passive, or otherwise while the undersigned or any participating child is in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with SwiftKick. The undersigned understands and agrees that SwiftKick is not required to provide insurance to cover the undersigned or such participating children in the event they suffer illness, injury, death, property loss, theft, or damage of any sort upon, or about the premises or any facilities or equipment therein or participating in any program affiliated to SwiftKick.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them for any loss, liability, damage, or cost they may incur due to the presence of the undersigned in, upon, or about the Woodmen Community Center premises or in any way observing or using any facilities or equipment of the Woodmen Community Center or participating in any program affiliated with the Woodmen Community Center whether caused by the negligence of the releasees or otherwise.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage or cost they may incur due to the presence of the undersigned in, upon or about Hanabi Augustine Albany Judo premises or in any way observing or using any facilities or equipment of Hanabi Augustine Albany Judo or participating in any program affiliated with Hanabi Augustine Albany Judo whether by the negligence of the releasees or otherwise.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any, loss, liability, damage or cost they may incur due to the presence of the undersigned or such children in, upon, or about TCYFCC premises or in any way observing or using any facilities or equipment of TCYFCC or participating in any program affiliated with TCYFCC whether caused by the negligence of the releasees or otherwise.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any, loss, liability, damage or cost they may, incur due to the presence of the undersigned or such children in, upon or about Pepperwood Foundation’s premises or in any way observing or using any facilities or equipment of the Pepperwood Foundation or participating in any program affiliated with the Pepperwood Foundation whether caused by the negligence of the releasees or otherwise.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releases (to include Sunny Hills High School and Fullerton Joint Union High School District) and each of them from any loss, liability, damage or cost they may incur due to the participation of the undersigned in the PTSA field trip to Disneyland/California Adventure sponsored, planned, and directed by the SUNNY HILLS HIGH SCHOOL PTSA.
THE UNDERSIGNED HEREBY. AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releases and each of them for any loss, liability, damage or cost they may incur due to the presence of the undersigned in, upon or about the Fitness Center premises or in any way observing, participating in personal training/fitness classes, or using any facilities or equipment of the Fitness Center whether caused by the negligence of the releases or otherwise.
